It covers the same core work as any accountancy service, but applied to the way healthcare income and costs behave. In practice that means Self Assessment for clinicians with employment, private or locum income, annual accounts and Corporation Tax for incorporated practices, bookkeeping that separates NHS and private streams properly, payroll for practice staff, VAT advice on the scope of the medical exemption, and tax planning around pension and structure questions. The difference is in the detail: knowing how associate agreements, partnership profit share, indemnity cover and CPD costs should be treated.
You can file a return yourself, but self-employment brings obligations that are easy to underestimate. You need to register with HMRC, keep records of income and expenses, work out taxable profit rather than total turnover, and account for payments on account that can catch people out in the first profitable year. Locum and associate work often means several payers and inconsistent income, which makes accurate record keeping and a properly prepared return particularly valuable.
Each source is treated according to its nature. The test is whether the cost was genuinely incurred for the business and is properly evidenced. Personal commuting, ordinary clothing and general living costs are not claimable, and mixed-purpose costs need apportioning.
Many medical services are exempt from VAT, but the exemption is not unlimited. Services that fall outside the scope of medical care, such as certain cosmetic treatments, some non-clinical products and specific advisory services, may be taxable. Where a business provides both exempt and taxable supplies, it may need to deal with partial exemption, which affects how much input VAT can be reclaimed. Registration thresholds, the value of taxable turnover and the nature of each service all need checking before any position is taken.
