For healthcare clients, the practical difference is that the accountant already understands how NHS pay, private fees, locum work, practice profits and clinical expenses fit together instead of treating each in isolation.
They suit anyone earning income from clinical or care work, whether that is a salaried GP with private consultation fees, a hospital consultant doing medico-legal reports, a dentist running a practice, a pharmacist, a nurse or carer with varying shift patterns, a physiotherapist or other allied health professional, a locum working through several agencies, or a clinic or care provider employing staff. Sole traders, partnerships and limited companies all fit within the service. If you are unsure whether your circumstances need specialist support, it usually comes down to the number of income sources, whether you employ people, whether VAT applies, and how much you want to handle yourself.
It depends on how your income arrives. Salaried employees taxed through PAYE may not need a return if that is their only income, but a return is generally required where you have untaxed income such as private fees, locum payments received gross, rental income, or significant dividend or savings income. A common example is a clinician whose NHS salary is taxed at source but who also receives private patient fees. There are also cases where a return brings a refund, for instance where professional expenses or pension contributions have not been captured through PAYE. Yes, and this is one of the most frequent situations we deal with. NHS salary is taxed through PAYE, while private fees, medico-legal work and similar income usually need to be reported separately through Self Assessment, often with payments on account to follow. Costs such as professional indemnity, registration fees, CPD and a proportion of home or vehicle use may be claimable against the self-employed element, and the apportionment needs a sensible basis. Because the two income streams affect your overall tax rate and the payments on account you are asked to make, it helps to plan the timing of work and payments rather than discover the position at filing time.
Common claims include professional body and registration fees, professional indemnity and medical defence subscriptions, CPD and required training, specialist equipment and its maintenance, uniforms and laundry where applicable, professional journals, business mileage, and a proportionate share of home costs where a space is genuinely used for practice administration. Limited company directors have a slightly different set of rules around what the company can pay for. The principle is that a cost must be incurred wholly and exclusively for the purposes of the profession or business, so mixed-use items need apportioning rather than claiming in full.
Yes. Many healthcare services are exempt from VAT, but that is not the end of the matter. Where a practice also supplies standard-rated services, sells goods, or incurs VAT on costs that relate to both taxable and exempt activities, then registration, partial exemption calculations and correct invoice wording all come into play. A clinic can also be required to monitor its VAT position even when it is not registered, particularly where income is growing. We can review what you supply, confirm where you stand, and handle ongoing VAT returns once the position is clear.
It is not compulsory, but the position for locums is rarely simple. Income may arrive gross from several sources, expenses need tracking properly, and engagement status questions can arise where a hospital or agency treats the work as employment. If you operate through your own limited company, there is the additional layer of director responsibilities, statutory accounts, Corporation Tax, PAYE for any salary you take, and a personal Self Assessment. An accountant helps ensure income is reported in the right place, that the structure still suits the way you work, and that deadlines are met without you having to track them yourself.
