The core work is familiar accounting and tax compliance, but applied to the way healthcare income is earned. That typically means preparing Self Assessment returns for sole traders, partners and locums, preparing annual accounts and Corporation Tax returns for limited companies and incorporated practices, running bookkeeping and payroll, handling VAT returns where a practice has taxable income, and advising on structure, drawings and tax planning. The difference is in the detail: separating NHS and private income, recognising the expenses that are genuinely allowable in clinical work, and understanding how sessional or locum arrangements affect tax.
It suits any healthcare professional or healthcare business with tax or reporting obligations. That includes GPs and practice partnerships, hospital doctors and consultants, locum and sessional clinicians, dentists and dental practices, pharmacists and pharmacy owners, nurses and advanced practitioners, physiotherapists and allied health professionals, and care providers, private clinics and medical practices. If your income comes from a single PAYE role and you have no other earnings, you may not need much beyond a straightforward tax check, but anyone with mixed income, a limited company, staff or VAT exposure will usually benefit from specialist support.
Not everyone does. If all your income is taxed through PAYE and there is nothing unusual to report, you may not need a return at all. A return is generally required where you have self-employed income from private practice or locum work, partnership income, untaxed income such as some report or advisory fees, higher-rate income from savings or dividends, or income above the threshold for child benefit charges. A short review of your circumstances is usually enough to establish whether a return is needed, and what needs to go on it if so.
NHS employment income is normally taxed through PAYE, with National Insurance deducted at source and pension contributions taken from pay. Private practice fees, locum earnings paid outside payroll and self-employed income are reported through Self Assessment, where income tax and Class 4 National Insurance apply to profits, and payments on account may become due. The two streams need to be recorded separately and supported by accurate figures, because combining them loosely makes it difficult to check that the PAYE already deducted has been properly credited against your overall liability.
Common claims include professional registration and body fees such as GMC, NMC, GDC or HCPC fees, professional indemnity insurance, continuing professional development and required training, specialist equipment and instruments used in your work, medical reference materials and journals, clinical uniforms where they are work-specific, and business travel or mileage between sites. Costs that are purely personal, such as ordinary clothing, routine commuting or general gym memberships, are not allowable. Where a practice or home office is involved, apportionment rules apply and need to be calculated sensibly.
Many medical and healthcare services are exempt from VAT, which is why some practices never register. However, not everything a healthcare business does is exempt. Non-medical treatments, cosmetic procedures carried out for non-medical reasons, retail sales, some training and medico-legal or report-writing work can be standard-rated, and income can quickly approach the registration threshold. Where a practice makes both exempt and taxable supplies, it can only recover a proportion of the VAT it pays on costs, and partial exemption calculations must be supported by good records. Registration is often not the right answer, so it is worth reviewing your position before a decision is made.

That means different contracts, fluctuating hours, pension auto-enrolment duties and, for NHS pension members, contributions and any annual allowance consequences. We can run payroll on your behalf, produce payslips and Real Time Information submissions, handle starters and leavers, and make sure your own remuneration is structured in a way that fits your wider tax position.
Yes. Most of the information we need can be shared digitally: bank statements, invoices, receipts, payroll data and practice reports. Cloud bookkeeping software makes it practical to keep records up to date from a consulting room, a clinic or home, and meetings can be held remotely at a time that fits around clinics and lists. What matters is the quality and timing of the information rather than where you are sitting when you send it.
