The core accounting work is familiar, but the context is not. Healthcare income frequently arrives from several sources at once, such as NHS salary, practice profit share, private fees, locum sessions and medico-legal work, and each may be taxed or reported differently. We map those income streams properly, check that deductions already made are correctly reflected, and make sure allowable healthcare-specific costs such as indemnity cover, registration fees and professional subscriptions are claimed where they are legitimately due. We also understand the VAT exemption rules that apply to medical care and the situations where they do not, which is a common source of confusion.
Many salaried clinicians have simple tax affairs and can manage alone. The picture changes when there is more than one income source, when expenses are being missed, or when tax codes have not caught up with locum work taxed at source. In those situations an accountant can reconcile the position, confirm that the right reliefs have been claimed and remove the risk of an unexpected bill or enquiry. Locums in particular often benefit from a second pair of eyes on their records and on whether their current structure still suits them.
We keep the streams separate so each is reported correctly, then bring them together into one accurate tax position. That means tracking practice or salaried income, recording private fees, noting any tax already deducted, and making sure pension contributions and other deductions are treated properly. For GP partners, this usually involves working from the partnership accounts and profit allocation, while for consultants and private practitioners it means reconciling fee income with practice costs and associated expenses.
It depends on the service and who provides it. Most medical care delivered by registered healthcare professionals is exempt from VAT, which means no VAT is charged on those fees and input VAT recovery is often restricted. However, not everything within a healthcare business is exempt. Aesthetic or cosmetic treatments, non-medical wellbeing services, retail sales, room hire and catering can be standard-rated. Where a business has a mix of exempt and taxable income, partial exemption rules apply and record keeping becomes much more important. We review your specific services and confirm how each should be treated.
Typical claims include professional indemnity insurance, GMC, NMC or GDC registration and retention fees, professional body memberships, continuing professional development, specialist equipment and instruments, clinical clothing and laundering, and a reasonable proportion of home costs where administrative work is genuinely done there. Practice-level costs such as clinical waste, software, insurance, utilities and staff costs are generally allowable business expenses. The key is that costs are incurred wholly and exclusively for the business and that the evidence exists to support them, which is why accurate record keeping matters more than any list of categories.
There is no universal answer, because it depends on your profits, how you take money out, your pension position, your appetite for administrative obligations and your plans for the future. Incorporation can offer tax advantages in some circumstances, but it also brings Corporation Tax returns, statutory accounts, Companies House filings and different rules on dividends and expenses. For some clinicians a partnership or sole trader structure remains more suitable. We look at your actual figures and circumstances before recommending a route rather than applying a general rule.
