For a locum GP, that might mean piecing together income from several NHS trusts and agencies for one tax return. For a dental practice, it might mean separating exempt healthcare supplies from standard-rated items on a VAT return, and running payroll for a mixed team of associates and support staff. The aim is to keep your filings accurate and on time, and to give you advice that reflects your clinical and commercial circumstances rather than a generic template.
Not always, but it is often worthwhile even in straightforward cases. Sole traders in healthcare typically have expenses that generalists may not think to ask about — professional registration fees, indemnity insurance, specialist equipment, CPD, professional subscriptions and travel between sites are common examples. There is also the question of whether your current structure is still the right one as your income grows, and whether you should be thinking ahead to VAT registration. A specialist accountant will ask the right questions early rather than discovering missed expenses or a structure problem at the end of the year.
NHS and private income often arrive through different routes and with different documentation: payslips and pension records from one, invoices and insurer statements from another. We compile them into a single coherent tax position, checking that any tax already deducted at source is properly credited and that expenses are allocated sensibly between the two income streams. Where a practice or practitioner is VAT-registered, the distinction matters commercially as well, because the VAT treatment of NHS and private services can differ. Broadly, expenses must be wholly and exclusively for the purposes of your work. In practice, that often includes professional body and registration fees, professional indemnity insurance, medical or clinical equipment and its maintenance, specialist clothing and uniforms, CPD and training that maintains existing skills, professional journals and reference materials, and travel between work locations that is not ordinary commuting. Some items, such as certain clothing or training that opens up a new career, are restricted, and home-office and vehicle claims depend on your specific circumstances. We review your spending each year and identify what is genuinely claimable, with the records to support it.
Many healthcare services are exempt from VAT, which can make the position less obvious than it seems. Registration becomes relevant once your taxable turnover from non-exempt activities crosses the registration threshold — and for practices, taxable turnover can arise from cosmetic treatments, retail sales, non-medical products or certain consultancy services. If you are registered and provide a mix of exempt and taxable services, you may need to apply partial exemption rules, which affect how much input VAT you can reclaim. VAT registration is not something to delay once the threshold is in sight, so it is worth reviewing your turnover position regularly.
