The technical work — accounts, returns, payroll, VAT — is the same discipline, but the context is different. Healthcare clients typically juggle several income sources at once, such as NHS salary, private fees, locum work and partnership profits, and each needs to be captured correctly on a tax return. They also deal with costs that general accountants may not recognise as claimable, including indemnity cover, professional registration fees, CPD and clinical equipment. VAT is another area where sector knowledge matters, because most clinical services are exempt while cosmetic treatments, retail sales and dispensing goods may be taxable. Working with an accountant who understands these distinctions reduces the risk of overpaying tax or triggering avoidable HMRC queries.
As a salaried doctor, your tax position is largely PAYE, with a Self Assessment return mainly to confirm payments and any additional income. As a partner, you are taxed on your share of the practice's profits rather than a salary, you pay tax and National Insurance through Self Assessment, and you may need to make payments on account during the year. Practice accounts, profit-sharing ratios, capital allowances on equipment and the treatment of practice expenses all become relevant. Partner drawings also need to be planned carefully, because tax is due on profits regardless of how much cash you have withdrawn.
Yes. Returns that combine NHS pay, private consultation or treatment fees, locum shifts, teaching or medico-legal work are common in healthcare. It depends on what you supply. Most medical and clinical care is exempt from VAT, and exempt income does not count towards the VAT registration threshold. If a business makes both exempt and taxable supplies, partial exemption rules may apply and input VAT on shared costs often needs apportioning. Payroll for a practice needs to cope with a mix of contracts: salaried clinicians on substantive posts, part-time and bank staff, reception and administrative teams, and locums who may be employees or self-employed depending on the arrangement. Pension arrangements differ across roles, and the status of each worker affects PAYE and National Insurance. That said, we are flexible about how you prefer to communicate — some clients want a scheduled review conversation, others just want to know the deadlines are handled. If you move between sites or work irregular hours, the process is built to fit around that rather than the other way round.
Switching is usually more straightforward than people expect. Once you decide to move, we can contact your existing accountant to request professional clearance and the handover of records, which typically covers your latest accounts, tax returns, VAT submissions if applicable and payroll information. You will also need to provide details of your income sources, any company or partnership information and access to your bookkeeping records.
