For a clinician or a practice, healthcare accounting covers considerably more than filing one tax return. It typically includes preparing annual accounts and company or partnership tax returns, Self Assessment for individuals with NHS and private income, bookkeeping, payroll for clinical and administrative staff, VAT advice on exempt and standard-rated services, pension contribution reporting and tax planning around how profits are taken from the business. The purpose is to keep records accurate, filings on time and your tax position understandable — so that decisions about taking on more private work, bringing in an associate or changing structure are made with real numbers behind them.
Yes. A self-employed physiotherapist, a single-handed dentist or a locum doctor working a few sessions a week has a different set of requirements from a multi-partner GP surgery or a clinic employing twenty staff, but the underlying needs are similar: correct records, accurate returns, deadlines met and advice that reflects how the income is earned. The scope of work is what changes. A sole practitioner might need Self Assessment, expense guidance and a simple bookkeeping routine. A larger practice will usually need payroll, VAT, management accounts and support for partners or directors.
Usually, yes. If you receive income that has not been taxed at source — private fees, locum payments made gross, consultancy income, rental or investment income, or you are affected by the high income child benefit charge — you will normally need to file a Self Assessment return. NHS salary taxed through PAYE does not remove that requirement when untaxed income sits alongside it. Filing also brings payments on account into play, which makes the timing of tax bills as important as the calculation itself, and that is where planning ahead genuinely helps.
Medical care provided by appropriately registered professionals is generally exempt from VAT, but the exemption does not cover everything. Cosmetic and aesthetic treatments, some screening and non-medical services, retail sales, room hire and certain equipment arrangements can be standard-rated. If you provide a mixture of exempt and taxable services, you may need to look at registration thresholds, partial exemption and how costs are apportioned between them. Getting this wrong in either direction tends to be expensive, so it is worth reviewing before you register or before you change the range of services you offer.
Common allowable expenses include professional body and registration fees, indemnity insurance, medical equipment and consumables, specialist clothing such as scrubs and uniforms, training that maintains existing professional skills, practice premises and home office costs where genuinely used for work, and business mileage. Expenses that are private or dual purpose, such as ordinary clothing or non-business travel, are not allowable. The test is whether the cost is incurred wholly and exclusively for your work and whether you hold evidence to support it. We review claims as part of preparing your accounts, so nothing is over-claimed and nothing useful is missed.
Yes. Limited companies and incorporated practices need annual accounts for Companies House, Corporation Tax returns, director Self Assessment, payroll and often VAT returns. Incorporated healthcare businesses also face decisions about salary versus dividends, employer pension contributions, associated company rules and how much profit to retain for equipment, premises or expansion. Yes. Practice payroll commonly involves salaried GPs, nurses, receptionists, practice managers and administrative staff on different contracts and pension arrangements, plus locums who may be engaged on a self-employed basis. We can run the payroll, calculate PAYE and National Insurance, handle pension auto-enrolment duties, issue payslips and prepare the reporting HMRC requires. Where locums are involved it is also worth checking whether off-payroll working rules apply to an engagement, because that changes who is responsible for operating the tax.
Yes. Almost all the information needed — bank statements, invoices, payroll records, pension figures and HMRC correspondence — can be shared securely online, and meetings can be held by phone or video. Remote working suits clinicians whose diary rarely allows time for an office visit, and it means advice can be arranged around clinic hours rather than the other way round. If you would rather meet in person or drop documents off, that can be arranged as well.
