On the advisory side it includes tax planning, reviewing whether your business structure still suits you, advising on allowable expenses, interpreting NHS pension and superannuation figures, and preparing management accounts that help partners or directors understand how a practice is performing. For healthcare clients, an important part of the role is understanding how different income sources interact — for example a salaried NHS post alongside private fees and a share of practice profits.
It can be, particularly once income comes from more than one source. Healthcare work often involves NHS pension figures such as annual and lifetime allowance considerations, practitioner pension arrangements, indemnity cover, exempt versus standard-rated supplies for VAT, and pay structures that differ from a typical limited company. A general accountant can certainly prepare a return, but specialist knowledge reduces the risk of misinterpreting a pension statement or treating an exempt supply as taxable, and it usually means fewer questions being sent your way.

Medical care provided by registered health professionals is generally exempt from VAT, but that does not remove the need to think about registration. Income from services that fall outside the exemption — certain cosmetic or aesthetic treatments, non-medical products, retail sales, equipment hire or mixed supplies — counts towards the taxable turnover threshold and can push a practice into a registration requirement. Once registered, partial exemption rules may apply, which affect how much input VAT can be reclaimed. Working through this before you reach a threshold is far easier than resolving it afterwards.
Payroll for a practice usually involves a mix of salaried clinicians, part-time staff, reception and administrative teams, and sometimes locums paid through the payroll. We manage the full cycle: calculating pay, processing starters and leavers, handling statutory payments such as sick pay, submitting Real Time Information reports to HMRC, and dealing with auto-enrolment pension duties and contributions. Where staff work variable hours or receive additional payments, we make sure those are recorded consistently so the year-end position is accurate and each employee receives the documentation they need.
Allowable expenses depend on your working arrangements rather than your profession, but common examples include indemnity and professional insurance, professional body subscriptions, continuing professional development and required training, specialist equipment and clinical clothing, and a reasonable proportion of home office or travel costs where genuinely incurred for work. The key is that the cost must be incurred wholly and exclusively for the purpose of the work, and the treatment differs between employed, self-employed and incorporated arrangements. We review expenses as part of preparing your return rather than assuming a standard set of claims applies.

Records can be shared securely online, bookkeeping software keeps information in one place, and meetings can be held by phone or video at times that fit around clinics and surgery hours. Many healthcare clients prefer this because it removes travel time from an already demanding week.
