Your accountant needs to understand how NHS contract payments, private fees, locum sessions, partnership profits and pension contributions fit together, which expenses are genuinely allowable for clinical work, and how to record everything in a way that respects patient confidentiality. Good healthcare accounting also includes advice: on structure, on timing of income, on payments on account, and on the questions that arise between filing deadlines.
Not everyone does, but many locums benefit from one. If you work through several agencies or practices, your income arrives in different formats and at different times, which makes it easy to miss income, misplace expense evidence or misjudge how much tax you will owe. An accountant can bring that together into a single accurate picture, claim the expenses you are entitled to — professional registration, indemnity cover, CPD, travel between sites and a proportion of home costs used for admin — and make sure your payments on account are set at a sensible level so you are not caught out by a larger bill than expected. These calculations require accurate pension savings statements and a good understanding of how your total income sits for the relevant year, so it is worth reviewing your position before the self-assessment deadline rather than after a charge appears.
Often, yes — although many healthcare services are exempt from VAT, that exemption does not cover everything. Services that fall outside the scope of the healthcare exemption, such as certain cosmetic treatments, non-clinical wellbeing services, retail sales or the supply of goods, may be standard-rated. Alongside the monthly calculations and payslips, there are real-time reporting obligations to HMRC and auto-enrolment pension duties for eligible staff, including assessing new employees, handling opt-ins and opt-outs, and making contributions on time. Keeping payroll and pension administration joined up with the practice accounts avoids duplication and reduces the risk of inconsistencies between what has been reported and what has been paid.
