NHS and private income are both taxable, but they arrive in different ways and are reported on different bases. Employed NHS income is usually taxed through PAYE, while self-employed private fees, locum work and some associate arrangements are reported on your tax return, with payments on account following once your liability exceeds the relevant threshold.
