Because healthcare income often comes from several directions — NHS pay, private fees, locum shifts, teaching or medico-legal work — a large part of the job is keeping each source correctly categorised so returns are accurate and you can see what your private work or practice is genuinely earning.

If your income is mixed, it is worth reviewing the position rather than assuming exemption covers every service you provide.

It depends on how the practice is set up, how profits are shared, and what the medium-term plans are for taking on or retiring partners. Partners are taxed on their share of profits through Self Assessment, while a limited company or LLP changes how profits can be distributed and how National Insurance applies. There are administrative and liability considerations alongside the tax ones, so a structured review of the options usually gives a more useful answer than a simple comparison of rates.
