The healthcare-specific part is understanding how NHS income, private fees, locum work, insurance payments and pension contributions interact, so nothing is missed or misreported.
They suit anyone earning income from clinical work or running a healthcare business. That includes GPs and locum GPs, hospital consultants with private practice, dentists and dental associates, pharmacists and pharmacy owners, nurses and advanced practitioners, physiotherapists, psychologists, optometrists, podiatrists and other allied health professionals, as well as partners in medical practices, clinic owners and care providers. If your income is straightforward salary only, you may need very little support. As soon as you have self-employed earnings, a partnership share, a limited company, employees or VAT-registered services, specialist help becomes far more valuable.
Yes, in most cases. If you receive self-employed income from locum work, private consultations, medico-legal reports, insurance work or similar, you will normally need to register with HMRC and file a Self Assessment return, even if you also have a salaried NHS or hospital role taxed through PAYE. The return brings all your income together and calculates the final liability, taking into account tax already paid. Deadlines for registration and filing are fixed, and late filing or payment can trigger penalties, so it is worth getting the position confirmed early rather than at the last minute.
NHS salary is usually taxed through PAYE, with tax deducted at source, while private fees, locum payments and similar income are typically self-employed earnings that you report yourself. Both feed into your overall tax position, and the PAYE already deducted is credited against your final bill. The two streams also behave differently for expenses, pension contributions and record keeping. Keeping them clearly separated in your accounts makes the year-end process far smoother and reduces the risk of questions about which costs relate to which income.
Commonly allowable costs include professional indemnity insurance, registration and licence fees for bodies such as the GMC or NMC, medical equipment and instruments, specialist clothing such as scrubs and uniforms, professional journals and subscriptions, CPD courses and conferences, and a reasonable proportion of home office, phone and travel costs where these relate to self-employed work. The underlying test is that the expense must be wholly and exclusively for the business. We review your spending annually so legitimate costs are captured and anything unlikely to be accepted is flagged before it goes into a return.
Many healthcare services are exempt from VAT, but not all activities are treated the same way. Private cosmetic work, some diagnostic services, retail sales in a pharmacy or clinic, and certain non-medical services can be standard-rated. If your taxable turnover across all your business activities crosses the registration threshold, registration may be required, and mixed-income practices need to consider partial exemption rules. Because exempt and standard-rated income behave differently, it is worth reviewing your position before you approach the threshold rather than after.
Yes. We can run payroll for the whole practice, covering salaried staff, associates and other workers, and handle payslips, pension auto-enrolment, starter and leaver reporting and real-time submissions to HMRC. We also look at how each worker is engaged. Employment status matters, because treating someone as self-employed when HMRC considers them employed can lead to unexpected tax and National Insurance liabilities. Getting the arrangement right from the outset is far less costly than correcting it later.
