Not every doctor does, but many benefit. If all of your income is taxed through PAYE and you have no significant additional income or expenses, your affairs may be straightforward. Once you take on locum shifts, private patient fees, consultancy work or medico-legal income, or you want to claim professional expenses such as medical indemnity and professional subscriptions, a Self Assessment return is usually required and specialist help becomes valuable. An accountant can also help with pensions, business structure and planning, which is often where the real value lies. Your position depends on your own circumstances, so it is worth reviewing rather than assuming.
Commonly considered costs include medical indemnity insurance, GMC and royal college or specialist society subscriptions, approved training and continuing professional development, specialist equipment and professional publications. Travel and mileage may be claimable in certain circumstances, but the rules focus on whether the cost was incurred wholly and exclusively for the professional activity. Some expenses are only allowable against certain types of income, and employment expenses are treated differently from business expenses. Because the detail matters, it is best to review your specific costs rather than rely on a general list.
Locum work usually means fees paid without tax deducted, so you will generally need to register for Self Assessment, keep records of every assignment and its income, and set money aside for the tax and National Insurance that will fall due. You may also need to consider whether you are working through an agency, as a sole trader or through a limited company, because each route affects how you are taxed and what records and filings are required. Mileage and travel between sites, equipment and indemnity cover are among the costs worth tracking carefully from the outset.
Your NHS salary is normally taxed through PAYE, while private fees and similar income often need to be reported through Self Assessment. The two are not separate tax systems — they combine when your overall tax position is calculated, which can mean tax is due on the private income at your marginal rate. Payments on account may also apply once a Self Assessment liability arises. Keeping the income streams clearly separated in your records makes the return far easier to complete and reduces the risk of errors.
Many doctors providing medical services are exempt from VAT, but the position is not automatic for every activity — some non-medical or commercial services, such as certain report writing or cosmetic treatments, may be treated differently. Where you provide a mix of services, or where your practice turnover grows, VAT registration may become relevant. The right approach depends on the nature of your work, so it is worth a specific review rather than a general assumption.
