Not every doctor does, but many benefit. If all of your income is taxed through PAYE and your affairs are straightforward, your position may be simple enough to manage alone. Where things become more involved — private medical income, locum fees, consultancy work, a practice partnership, medical indemnity and subscription claims, or pension questions — specialist accounting support usually pays for itself by keeping your reporting accurate and your expense claims properly evidenced. Whether you need help depends on your income sources, employment status and how much of your earnings are taxed at source.
Your NHS salary is normally taxed through PAYE, but private fees, locum payments and consultancy income often are not. If you have untaxed income above the reporting threshold, you generally need to register for Self Assessment and declare all of it on one return, which means your PAYE income and your private earnings are brought together when your overall tax position is calculated. Because tax may already have been collected on part of your income, it is important that the return is prepared accurately so you are not paying twice or underpaying. Your individual position depends on your total income and circumstances.
There is no single list that applies to everyone, but common areas include medical indemnity insurance, professional body subscriptions, revalidation and CPD costs, specialist equipment used for your work, and business travel or mileage where journeys are not ordinary commuting. The key test is whether a cost is incurred wholly and exclusively for the purposes of your professional work. Some expenses are straightforward, while others, such as mixed-use items or costs relating to both employment and private practice, need more careful treatment. We review your spending and advise on what can reasonably be claimed.
Yes. Locum work brings its own challenges: income may come from several practices or agencies across the year, travel between sites needs to be tracked, and there is usually no employer handling your tax through PAYE. We can help you keep clear records of shifts, fees and expenses, prepare your Self Assessment return, and advise on whether your working pattern makes a particular business structure more suitable. What works best depends on how regularly you work, your contract terms and your overall income.
Sometimes, but not automatically. Incorporation can be worth considering for certain private practice, consultancy or medico-legal arrangements, and there are potential advantages alongside additional obligations such as company accounts, Corporation Tax returns, payroll and stricter rules on how money is taken out of the company. For doctors whose income is largely NHS employment or who work as a traditional practice partner, a limited company is often not the right fit. The decision depends on your contract terms, income sources, tax position and longer-term plans, so it is worth reviewing properly before making a change.
At a minimum, keep records of all income received, invoices issued, bank and card statements, and receipts or evidence for expenses you intend to claim, including indemnity cover, subscriptions, training costs and mileage logs. Good records make your return more accurate and easier to support if questions arise later. Keeping digital copies and using cloud accounting software can simplify the process considerably. Records generally need to be retained for a set period after the relevant tax year, and we can confirm what applies to your situation.
Many doctors prefer to handle things around clinical commitments, so remote working is usually straightforward. Documents can be shared securely, records kept in cloud accounting software, and conversations held by phone or video at times that fit around clinics and on-call rotas. If you would rather meet in person, that can be arranged as part of the process, but it is not a requirement for receiving full accounting and tax support.
It is usually simpler than people expect. Once you decide to move, we contact your existing accountant for the professional clearance and handover information needed, including copies of previous accounts, returns and records. You do not need to manage that conversation yourself or explain your reasons. After that, we review your position, confirm what is outstanding and take over the ongoing work, making sure nothing is missed in the transition.
