It depends on your circumstances, but many doctors benefit from professional support. If your income comes solely from NHS employment and is taxed through PAYE with no untaxed income, your position may be relatively straightforward. However, doctors who earn locum income, private practice fees, consultancy income, or who have investments and property income alongside their salary usually need to file a Self Assessment tax return, and the interaction between those income streams can be easy to get wrong. An accountant helps ensure the return is complete and accurate, that allowable expenses are claimed correctly, and that any planning opportunities are considered.
Commonly considered expenses include medical indemnity insurance, GMC and royal college or specialist society subscriptions, and certain training and professional development costs. Mileage for practice-related travel, specialist equipment, professional books and journals, and a proportionate share of home office costs may also be relevant in some circumstances. The rules differ between employment and self-employment, and some costs that feel professional in nature are not allowable. Because eligibility depends on the specifics of your role and how the expense was incurred, we review each item on its own merits rather than applying a blanket list.
Locum work is usually treated as self-employment if you work on your own account and invoice practices or agencies directly, though arrangements vary and some locum engagement is taxed through PAYE. Where you are self-employed, you generally report income and allowable expenses through Self Assessment, and you may need to consider whether registration for VAT is required. Keeping accurate records of each booking, the fees invoiced, agency deductions and travel between sites makes the year-end position far easier to establish.
Yes — a Self Assessment tax return can report employment income from the NHS alongside self-employed private practice or consultancy income, and any other income you receive. The important part is that each source is reported under the correct category and supported by accurate records, because employment expenses and self-employment expenses follow different rules. Where private income is substantial, it may also be worth reviewing whether operating through a limited company or partnership would be more appropriate.
It can be beneficial for some doctors, but it is not automatically the right answer. A limited company brings Corporation Tax, statutory accounts, payroll and additional filing obligations, and the tax outcome depends on how profits are extracted, your other income, and your pension position. For doctors with modest or occasional private income, self-employment is often simpler and equally effective. We look at your actual numbers and working patterns before recommending a structure.
VAT registration depends on the nature and level of your income, and some medical services are exempt while others are not. This is a technical area, so it is worth reviewing before you assume either that registration is unnecessary or that it is required. If private income has grown, or if you are generating significant consultancy or medico-legal fees, we can assess your position and handle registration and returns if they apply to you.
NHS pension contributions reduce your taxable pay, and pension growth can also interact with annual allowance considerations, particularly for doctors with high or variable earnings. Because pension rules and limits change, and because your position depends on your total income and contribution history, this is an area where forward planning is usually more valuable than reacting after the tax year has ended.
