Not every doctor does. A salaried doctor with a single employer, straightforward tax code and no other income may manage perfectly well alone. The position changes once private fees, locum work, consultancy, pension income or a partnership share come into play, because there is more to declare, expenses to claim correctly, payments on account to budget for and deadlines to track. An accountant can help make sure your return reflects your actual circumstances and that you are claiming the reliefs you are entitled to, while giving you a clearer view of what is owed and when.
Yes — this is one of the most common situations we deal with. NHS salary is usually taxed through PAYE, while private fees, medico-legal work and some locum income may need to be declared through Self Assessment. Bringing both together in one return helps ensure the correct amount of tax is paid across the year and reduces the risk of an unexpected bill later. We can also look at how expenses and pension contributions relating to each income source interact, since the treatment is not always identical.
Potentially allowable costs include medical indemnity insurance, GMC and royal college or specialist society subscriptions, CPD and training that maintains or updates existing skills, specialist equipment and instruments, uniforms, professional journals and mileage for business travel. Whether an expense can be claimed depends on your employment status and how the cost relates to your work, so it is worth reviewing your outgoings rather than assuming either way. Locum work can be engaged in different ways, and the tax treatment follows the arrangement. Some locums work through an agency or umbrella company on a PAYE basis, while others are treated as self-employed and need to file a Self Assessment return, account for their own expenses and budget for tax payments on account. Travel between temporary assignments, equipment and indemnity costs often feature in the figures. Because employment status questions sit behind all of this, it is worth getting your position reviewed rather than relying on how a previous contract was structured.
It depends entirely on your circumstances. A limited company can suit some private practitioners, particularly where profits are retained in the business or where income is shared, but it also brings company accounts, Corporation Tax filings, payroll obligations and additional administration. A partnership or sole trader arrangement may be more appropriate for others, especially where NHS pension implications are involved. We look at your income levels, working pattern, pension position and long-term plans before recommending a structure, and we explain the trade-offs rather than presenting one option as universally better.
NHS Pension Scheme contributions are deducted from pay and attract tax relief, but they still count towards your annual allowance position, which can become relevant where private income increases your overall earnings or where your pension growth is higher than expected. Private pension contributions you make personally may also attract relief, subject to the rules that apply to your circumstances. Because these areas interact with your tax bands and can trigger additional charges, they are worth planning for rather than revisiting after the tax year ends.
