Not every doctor does, but many benefit from one. If your only income is a salaried NHS post taxed through PAYE and you claim nothing beyond the standard treatment, your tax affairs may be straightforward. The picture changes once you have private fees, locum income, consultancy work, a practice interest or untaxed income that requires a Self Assessment return, because those streams need to be reported accurately and supported by records. An accountant can also review whether expenses such as indemnity cover, professional subscriptions or training costs are being claimed appropriately, and help you plan for payments on account rather than being caught out by them.
Yes. We bring together employed NHS income, private medical fees, locum earnings and consultancy income into a single, accurate return, making sure the correct figures are reported under the right headings and that any tax already deducted at source is properly accounted for. We also check that allowable professional expenses are supported and correctly treated, and we look at how the resulting liability interacts with payments on account so you understand what will be due and when.
Locum work usually brings a wider range of potentially allowable costs than a permanent post, but the rules still depend on your circumstances. Common areas we review include travel and mileage to temporary placements, professional indemnity insurance, registration and subscription costs, training and professional development, specialist equipment, and a proportion of costs where you work from home in a genuine business capacity. Ordinary commuting between home and a regular workplace is generally not allowable, so the detail of where you work and how often matters. We can assess your own position rather than applying a blanket list.
It depends on the nature of the cover, your employment status and how the cost relates to your work. Indemnity cover required for your medical practice may qualify for relief in certain circumstances, and subscriptions to professional bodies that HMRC recognises for tax purposes can also be allowable. Because the treatment differs between employed and self-employed doctors, and between different types of cover, we look at what you actually pay for before advising on how it should be treated in your return.
There is no universal answer, and it is not simply a question of which structure sounds most tax efficient. A limited company can suit some doctors with substantial private or locum income, but it brings statutory accounts, Corporation Tax, payroll and dividend administration, plus off-payroll working considerations where you contract through an intermediary. Sole trader or partnership arrangements are simpler to run and may be perfectly appropriate. We review your income levels, working patterns, pension position and future plans, then explain the practical trade-offs before you decide.
Many medical services provided by qualified professionals are exempt from VAT, but not everything a doctor does falls within that exemption. Certain medical reports, cosmetic treatments, medico-legal work and non-medical services may be treated differently. Registration is generally driven by whether your taxable turnover crosses the registration threshold, which means the answer depends on the mix of your income rather than the fact that you see private patients. We can review your activities and advise on whether registration is required and how it should be handled.
