Not every doctor does. If your only income is a salary taxed through PAYE and you have no side income, no significant expenses to claim and no complexity in your affairs, you may not need ongoing support. The picture changes if you have private fees, locum or consultancy income, a partnership interest, a limited company, rental income or higher earnings that bring pension and allowance questions into play. Where you have more than one income source or an ongoing Self Assessment obligation, having an accountant review your position can help you avoid errors and missed reliefs.
Yes. Combining NHS salary with private medical income is a common situation, and the main challenge is making sure both are reported accurately in the right places on your return. We gather the figures from your payslips and P60 alongside your private income records, account for any tax already paid through PAYE, and prepare the return so your overall liability is calculated correctly. Where you have expenses relating to your private work, we look at whether they are allowable before including them.
It depends on your employment status and the nature of the cost. Medical indemnity insurance, professional body subscriptions, relevant training and professional development, equipment, and business travel or mileage can all be candidates, but the rules differ between employment and self-employment, and a cost must generally be incurred wholly and exclusively for the purposes of the work. Some expenses are restricted or not allowable at all. We review what you have spent and advise on what can properly be claimed rather than making assumptions.
Locum work is often paid through invoices rather than payroll, which usually means you are responsible for reporting that income and paying tax and National Insurance yourself. Depending on how you work, you might operate as a sole trader, a partner or through a limited company, and each route carries different accounting and filing obligations. We can help you decide on an appropriate structure, keep records of your income and expenses, and deal with the returns or accounts that follow from it.
There is no single answer, because it depends on your level of private income, how you are engaged by clients, how much you need to draw from the business, and what you want from your pension and wider tax position. Incorporation can bring different administrative obligations, including accounts, Corporation Tax and payroll, and in some cases it may not be advantageous at all. We look at your circumstances and explain the practical implications of each option so you can make an informed decision.
VAT registration depends on your turnover and the nature of the services you provide, and healthcare services are treated differently from some other activities. Because the position varies, it is worth reviewing before you approach any registration threshold rather than after. We can look at your income streams and advise on whether registration is required or appropriate, and handle VAT returns if you are registered.
We can work with doctors remotely, sharing documents electronically and communicating in whatever way suits your schedule, including around clinical commitments. If you already have an accountant, moving across is usually straightforward — we ask for your authorisation to act and request the relevant information from your existing adviser, then review your position before your next filing deadline. It helps to start the process with some time to spare before a return or accounts deadline falls due.
It depends on your circumstances, but typically we ask for details of your income sources, your payslips and P60 or P45 if you are employed, records of any private, locum or consultancy income, a summary of expenses you believe are relevant, and information about any business structure, partnership or company you are involved in. If you are registered for Self Assessment, your Unique Taxpayer Reference is also needed. Once we understand what applies to you, we will set out a clear list of what to send and when.
