If you are a salaried doctor with a single NHS employment, PAYE may already be handling most of your tax, although higher income, pension annual allowance issues or additional income can still make advice worthwhile. Once you take on private fees, locum work, consultancy, a partnership share or a limited company, the position becomes far more involved, and an accountant helps ensure returns are accurate, expenses are claimed correctly and deadlines are met.
Your NHS salary is taxed through PAYE, but private fees, locum income and consultancy earnings generally need to be reported through Self Assessment. The return brings both together, and HMRC calculates any additional tax due on the self-employed element. Because PAYE only accounts for your employment income, it is common to under-budget for the balancing payment, so planning ahead matters. The exact treatment depends on your income levels and circumstances.

Locum doctors usually need a reliable record of invoices raised, timesheets worked, and the travel and mileage associated with assignments, along with any equipment, indemnity or subscription costs. Because locum income fluctuates, it also helps to set money aside for tax as you earn rather than waiting until the filing deadline. Registering for Self Assessment, keeping income and expenses separate from personal spending, and reviewing payments on account each year all make a real difference.
There is no universal answer — it depends on your level of private income, how clients engage you, your pension position and your wider financial goals. A limited company can suit some doctors offering consultancy or medico-legal services, but it brings additional obligations including annual accounts, Corporation Tax, and potentially payroll and VAT. Pension contributions can be a valuable and tax-efficient part of your financial planning, but for higher-earning doctors the annual allowance, carry forward rules and the interaction between NHS pension scheme contributions and private income can create complications, including potential tax charges.
