Not every doctor does. If you are solely employed on PAYE with no additional income, no private work and no significant expenses to claim, your tax affairs may be manageable without professional help. However, most doctors with locum income, private fees, consultancy work, medico-legal income, a practice partnership or a limited company have reporting obligations that benefit from specialist support. An accountant can help you declare income correctly, identify expenses you may be entitled to claim, and plan ahead rather than discovering an issue after the filing deadline.
Your NHS salary is normally taxed through PAYE, but private fees, locum earnings and consultancy income are generally self-employment or untaxed income that must be reported through Self Assessment. The two sit side by side in one return, and the tax already deducted through PAYE is taken into account when your overall liability is calculated. Getting this right depends on keeping accurate records of private income and related expenses, and on understanding which costs relate to which activity. We can prepare and file the return and explain how the figures fit together.
Commonly claimed items include medical indemnity or professional indemnity insurance, professional body subscriptions, registration fees where they are required for your work, relevant training and continuing professional development, specialist equipment, and business travel or mileage between work locations. Whether a particular cost can be claimed depends on your employment status, whether the expense was incurred wholly and exclusively for your professional or business activities, and whether it has already been reimbursed. Because the rules vary, it is worth reviewing your specific costs rather than assuming a standard list applies.
Yes. Locum work often produces irregular income from multiple sources, along with travel between sites and periods where no work is booked. We can help you keep track of earnings and expenses across contracts, record mileage and travel properly, and report the resulting profit correctly on your Self Assessment return. Where locum work is combined with salaried NHS employment or private practice, we look at the whole picture rather than treating each element in isolation.
There is no single answer that suits every doctor. Sole trader or partnership arrangements are common for private practice and consultancy work, while some doctors use a limited company, which brings different reporting requirements such as statutory accounts, Corporation Tax and payroll obligations. The right choice depends on your income levels, how many people are involved, how profits are shared, your pension position and your longer-term plans. We can talk through the implications of each structure so you can make an informed decision.
VAT is not automatically applicable to all medical services, and the position depends on the nature of the services you provide and your overall taxable turnover. Some medical services are exempt, while others may not be. If your private or consultancy income grows it is worth reviewing whether registration becomes necessary and how it would affect your pricing and record keeping. We can assess your circumstances and help with registration and returns where VAT applies.
Yes. Much of the accounting work we carry out for doctors is handled through secure digital document sharing, email and video or telephone calls, so you are not required to attend in person for routine matters. That said, some doctors prefer a mix of remote contact and scheduled reviews. What matters most is that your records are organised, your questions are answered clearly, and your filing obligations are met on time.
Changing accountants is usually far simpler than doctors expect. We can contact your existing accountant on your behalf to request the records and information we need, subject to your authorisation, and we will confirm the position with HMRC so that correspondence comes to us. If your previous returns were filed some time ago, we will review the information to understand your current position before we take anything on. From there, we agree what support you need and put it in place.
