Not every doctor does — a salaried hospital doctor with no other income may find that their tax is handled through PAYE and needs little intervention. However, once you add locum shifts, private practice, consultancy work, untaxed income or a limited company, the position becomes much less straightforward. An accountant who understands medical income can help ensure everything is reported correctly, that relevant professional expenses are claimed, and that structure and planning decisions are made with the full picture in mind.
It depends on your circumstances. PAYE usually collects the right amount of tax on a straightforward NHS salary, but a return is often required where you also have self-employed income from locum work or private practice, significant untaxed income, higher-rate income from savings or dividends, or certain pension situations. High earners may also need to file where child benefit or pension annual allowance charges are in point. We can confirm whether a return is required in your case.
NHS salary is normally taxed through PAYE, while private practice, locum and consultancy income is usually reported as self-employment on a Self Assessment return unless it is paid through a limited company. The two are not mutually exclusive, and it is common for doctors to have both. The key is making sure the self-employed figures are accurate, that expenses are attributed correctly, and that tax already paid through PAYE is properly credited against your overall liability.
Potentially allowable items often include medical indemnity insurance, professional body subscriptions, registration fees, relevant training and continuing professional development, specialist equipment, and business mileage or travel undertaken in the course of your work. Ordinary commuting is generally not allowable. Whether a particular cost qualifies depends on your employment status, whether the expense was incurred wholly and exclusively for the purposes of your work, and how it is evidenced, so it is worth reviewing your specific outgoings rather than assuming.
There is no universal answer. A limited company can suit some doctors with substantial private or locum income, offering a different tax and National Insurance treatment and a clearer separation between personal and professional finances, but it brings additional compliance obligations such as statutory accounts, Corporation Tax returns and payroll responsibilities. For lower levels of private income, sole trader status may be simpler and perfectly adequate. The right choice depends on your income levels, plans, and how much administration you want to take on.
Many medical services provided by registered health professionals are exempt from VAT, which means VAT registration is not always required or beneficial. However, the position can differ where you provide non-exempt services, sell goods, or operate through a structure that includes other activities. Because exemption rules are specific, we review what you actually provide before advising on registration and returns.

Much of the accounting process can be handled remotely — documents and records can be shared securely, and meetings can take place by phone or video at times that fit around clinics and on-call commitments.
