If all your income is taxed through PAYE and you have no private earnings, no significant expenses and no pension complications, your position may be manageable on your own. But as soon as you take on private consultations, locum shifts, consultancy work, a practice role or multiple income sources, the reporting becomes more involved. An accountant can help ensure everything is declared correctly, that allowable professional expenses are claimed, and that you are not paying more tax than necessary. Many doctors find the reassurance and time saved are worth it well before their affairs become complicated.
Your NHS salary is usually taxed through PAYE, but if you also receive private fees, locum income or consultancy payments, those amounts generally need to be reported through Self Assessment. You would normally need to register for Self Assessment, keep records of your untaxed income and related expenses, and complete a return each year. The PAYE already paid on your salary is taken into account, and any additional tax due on the self-employed element is calculated from there. The exact position depends on whether you are employed, self-employed or both in each role, so it is worth reviewing your specific arrangement.
Commonly claimed costs include medical indemnity insurance, professional body and registration fees, mandatory training and continuing professional development, specialist equipment and uniform costs, and business mileage for travel between work locations. Whether a particular expense qualifies depends on it being incurred wholly and exclusively for your work, and some items are treated differently depending on whether you are employed or self-employed. Claiming incorrectly can cause problems, and missing legitimate expenses means paying more than you should, so it is worth reviewing your costs properly.
Locum work is often treated as self-employment, which means you would typically register with HMRC, report your income and expenses through Self Assessment, and pay tax and National Insurance based on your profits. If you work through an agency, the way you are engaged matters, because some arrangements involve PAYE rather than self-employment. Locums also have to think about record keeping across multiple contracts, travel between sites, indemnity cover and pension implications. Because the correct treatment depends on how each engagement is structured, it is worth checking rather than assuming.
Not automatically. A limited company can suit some doctors with substantial private income, offering a different approach to how profits are extracted and how tax is paid, but it also brings additional obligations such as statutory accounts, Corporation Tax returns, payroll and director responsibilities. For doctors with modest private earnings, sole trader status is often simpler. The right choice depends on your income level, how the income arises, your pension position and your longer-term plans. It is a decision worth discussing before incorporating rather than unpicking afterwards.
Yes. Partnership accounting for medical practices involves preparing partnership accounts, allocating profits between partners in line with the agreement, and preparing each partner's individual tax return. We can also look at capital accounts, drawings, practice expenses, and how the partnership interacts with any private work each partner carries out. Because partners often have different additional income sources and pension arrangements, the work involves coordinating the firm's figures with each individual's wider tax position.
Many medical services provided by registered health professionals are exempt from VAT, which means VAT is not charged on those fees and input VAT on related costs is generally not recoverable. However, the position can become less clear where a practice has mixed income, provides non-medical or cosmetic services, or has significant costs attached to exempt and taxable activities. Registration thresholds and partial exemption rules may become relevant. We can review your specific activities and income mix before any decision is made.
Changing accountants is usually straightforward. We would contact your current accountant with your authority to obtain the handover information, including prior returns, accounts and records, and then review what has been done to date.
