Not every doctor does. If your only income is a straightforward NHS salary taxed through PAYE and you have no untaxed income, savings interest above the reporting threshold, or complex pension position, you may not need one. However, an accountant becomes valuable as soon as you take on locum shifts, private practice fees, consultancy or medico-legal work, join a partnership, or run a limited company. At that point the reporting requirements, expense rules and interactions between income sources are easier to manage with professional support.
It depends on your circumstances. Many salaried doctors with only PAYE income do not need to file, although HMRC may still issue a return if there is another reason, such as high income, untaxed savings or a pension charge. Doctors with self-employed income from locum work, private practice or consultancy generally do need to report it. If you also receive NHS income as an employee, that will usually be taxed through PAYE, but the self-employed side still needs to be declared in full.
NHS employment income is normally taxed through PAYE, with tax deducted at source, so it appears on your tax return but is generally not taxed again. Private medical income, locum fees and consultancy earnings are usually self-employed income, reported through Self Assessment, and no tax is deducted at source. That means you are responsible for calculating and paying the tax due, often through payments on account. Because the two income types are reported together, accurate records from both are essential.
Medical indemnity or defence union subscriptions are commonly treated as allowable expenses where they are required for your professional work. Professional body subscriptions and mandatory registration fees can also qualify, along with required training and professional development costs. Whether a particular cost is allowable depends on your employment or self-employed status, the reason for the expense and whether it relates to your existing role or a new one. It is worth checking before assuming a cost can be claimed.
Locum income is usually self-employed, so you need clear records of every shift or placement worked, the fees invoiced and the payments received. Keep timesheets, agency statements, bank records and details of expenses such as travel to temporary workplaces, indemnity cover, professional subscriptions and any equipment you have purchased for the work. Records of mileage for locum travel are particularly important, as the amounts can add up significantly over a year.
There is no single right answer. A sole trader structure is simple and often suitable when private income is modest, while a partnership may fit doctors running a practice jointly. A limited company can suit higher or more consistent private income, but it brings additional obligations such as annual accounts, Corporation Tax returns, payroll requirements and the need to consider how you extract income tax-efficiently. The decision should be based on your income levels, plans and personal circumstances.
VAT registration depends on your taxable turnover and the nature of your services. Some medical services are exempt from VAT, which can affect whether registration is compulsory or beneficial, while other activities may be treated differently. If you provide private services, run a practice with additional commercial activities or are approaching the registration threshold, it is worth reviewing your position rather than assuming either way.
Changing accountants is usually straightforward. Once you decide to move, your new accountant can contact your current one to request professional clearance and the handover of records, including copies of previous tax returns, accounts and bookkeeping files. Bringing your records up to date first helps the transition run smoothly, and we can help you identify anything outstanding so nothing is missed between firms.
