It depends on your circumstances. If all your income is taxed through PAYE and you have no additional income, pensions complications or significant expenses to claim, an accountant may not be essential. However, most doctors find that at least one element of their finances is more complex than it first appears, whether that is private fees, locum income, a practice partnership, a limited company, or the interaction between NHS pension contributions and other earnings. An accountant is usually valuable where income needs to be declared, expenses need to be claimed correctly, or decisions about structure and planning are being made.
Your NHS salary is taxed through PAYE, but private fees, locum payments and consultancy income are generally not, which means they usually need to be reported through Self Assessment. The two streams are brought together on your return, and any tax already paid through PAYE is credited against your total liability. The exact position depends on how much you earn, how the private work is structured, and whether expenses are being claimed, so it is worth reviewing rather than assuming.
Commonly claimed items include medical indemnity insurance, professional body subscriptions, required training and continuing professional development, specialist equipment and some travel costs. Whether a particular expense is allowable depends on it being incurred wholly and exclusively for your work, and the rules differ between employment and self-employment. Records matter here: claims are far easier to support when receipts and mileage logs are kept as you go.
There is no single right answer. A limited company can suit doctors with substantial private or locum income who want to control how profits are extracted, but it brings additional obligations including annual accounts, Corporation Tax, confirmation statements and possibly payroll. Operating as a sole trader is simpler and often adequate for smaller or occasional earnings. The decision should consider your level of income, your other employment, pension implications and your longer-term plans.
Partnerships need accounts that allocate profits between partners in line with the partnership agreement, together with each partner's individual tax return. Other areas often include bookkeeping for practice income and overheads, payroll if the practice employs staff, VAT registration where relevant, and clear reporting so partners can see how drawings and profit shares are calculated. Practice finances work best when they are reviewed regularly rather than at the last minute.
NHS pension contributions are typically deducted from your pay, while private income is taxed separately but sits within your overall income position. The combination can affect the tax relief you receive on contributions and may raise questions about pension contribution limits, particularly where private practice profits are significant. Because the rules are detailed and depend on your own figures, this is an area where personalised advice is worthwhile.
