If all of your income is taxed through PAYE and you have no untaxed income, expenses to claim or other complications, your tax affairs may be straightforward. Doctors who usually gain the most are those with locum or private income, those claiming professional expenses such as medical indemnity or subscriptions, GP partners and practice owners, and anyone with multiple income sources. The starting point is always your own circumstances rather than a general rule.
It depends on your income. Salaried doctors whose tax is fully collected through PAYE may not need to file, but a return is usually required if you have self-employed locum or private income, untaxed income from other sources, significant investment or rental income, or high income where additional tax is due. HM Revenue & Customs will normally tell you if they expect a return. If you are unsure, it is worth checking before the deadline rather than after it.
NHS employment income is generally taxed through PAYE, with tax deducted at source, while private practice or consultancy income is usually self-employed income reported on a Self Assessment tax return, with tax and National Insurance paid separately. Where you have both, they appear on the same return but are calculated under different rules, and expenses must be attributed to the correct income stream. Keeping those records separate throughout the year makes the return far more straightforward.
Locum doctors who are self-employed can often claim expenses that are wholly and exclusively incurred for their work. These may include medical indemnity insurance, GMC and specialist registration fees, professional body subscriptions, mandatory training and continuing professional development, equipment and specialist clothing, and travel between assignments, including a mileage claim where a personal vehicle is used. The rules are specific and not every cost qualifies, so it is worth reviewing your expenditure rather than assuming everything is allowable.
