Not every doctor does. If all of your income is taxed through PAYE and you have no untaxed income, self-employment, partnership profits or significant expenses to claim, your tax position may be straightforward. However, many doctors do need support: those with private practice income, locum work, consultancy or medico-legal fees, GP partners, doctors with a limited company, and anyone whose PAYE code does not collect the right amount of tax. Doctors with higher earnings and pension considerations can also benefit from planning rather than a purely reactive approach.
It depends on your circumstances. A salaried doctor whose tax is fully collected through PAYE may not need to file, although there are situations where HMRC still expects a return. You will typically need to file if you have self-employment or locum income, partnership profits, untaxed savings or investment income above the reporting limits, higher-rate income that has not been fully taxed, or income from property or other sources. If you have recently started private or locum work, it is worth checking your position rather than assuming PAYE covers everything.
NHS salary is taxed through PAYE as employment income, and tax is deducted at source. Private practice fees, locum payments made outside PAYE and consultancy or medico-legal income are usually treated differently — often as self-employed income, which means reporting on a Self Assessment return and potentially paying tax through payments on account rather than monthly deductions. The two can also interact, for example where expenses relate to both streams or where pension contributions are affected by overall earnings. Getting the treatment right for each source is important.
Commonly claimed items include medical indemnity insurance, professional body subscriptions and registration fees, professional training and development that maintains or updates your existing skills, specialist equipment needed for your work, and business travel or mileage where it is not ordinary commuting. Whether an expense is allowable depends on the rules and on your specific role — for example, a course that qualifies you in a new specialty is treated differently from one that updates existing knowledge. Claims should be proportionate and supported by records.
Medical indemnity cover and professional subscriptions relating to your work are commonly allowable where they are incurred wholly and exclusively for the purposes of your profession. For employed doctors claiming against employment income, the rules are stricter than for self-employed income, and expenses generally need to be incurred necessarily in the performance of your duties. Because the treatment can differ between NHS employment, private practice and locum work, it is worth checking how each cost should be claimed before filing.
Locum work is often treated as self-employment, which means registering with HMRC, keeping records of income and expenses, and reporting profits through Self Assessment. Some locum placements are paid through PAYE instead, in which case the tax is deducted at source — but you may still need to file a return. Locums frequently work through a limited company or an agency arrangement, and each structure has different implications for tax, National Insurance and administration. Mileage between placements, equipment and indemnity costs are among the expenses commonly relevant to locum doctors.
There is no single answer. A limited company can suit some doctors — particularly those with substantial private, locum or consultancy income — because profits are taxed through Corporation Tax and income is drawn as salary, dividends or a combination. It also brings additional obligations: annual accounts, confirmation statements, payroll, and in some cases VAT registration. For other doctors, working as a sole trader or within a partnership is simpler and equally appropriate. The right choice depends on your income levels, working pattern and plans, and it is worth reviewing periodically rather than setting up a structure and leaving it untouched.
Usually less than people expect. We typically ask for details of your current arrangements and permission to contact your existing accountant, then handle the professional clearance and request the records we need — accounts, tax returns, bookkeeping files and any correspondence with HMRC. If deadlines are approaching, we can prioritise those first. Doctors moving from another accountant often use the change as an opportunity to tidy up records, review their business structure and check that expenses and income have been treated correctly in previous years.
