Not every doctor does. If your only income is a straightforward NHS salary taxed through PAYE and you have no untaxed income or complex expenses, your tax may largely take care of itself. The picture changes once you have private medical income, locum or consultancy earnings, a limited company, a partnership interest or significant professional expenses. At that point, an accountant can help make sure income is reported correctly, allowable expenses are claimed appropriately and deadlines are met.
It depends on your circumstances. Many doctors with only a salaried NHS role do not need to file, but a return is usually required if you have untaxed income such as private fees, locum earnings, consultancy income, rental income or significant savings and dividend income. Higher earners may also be brought into Self Assessment through income thresholds or child benefit charges. We can review your position and confirm whether a return is required.
NHS salary is normally taxed through PAYE, with tax deducted at source. Private medical income is often treated as self-employment or, where a limited company is involved, through company profits and any salary or dividends you take. The two sit side by side on your tax return, and your tax position reflects the combined total. Because expenses and allowances can apply differently to each stream, keeping them clearly separated in your records helps.
Medical indemnity insurance and professional body subscriptions are commonly claimed by doctors, and for many they are a significant annual cost. Whether a particular payment is allowable depends on your employment or self-employment position and how the cost relates to your work, so it is worth checking rather than assuming. Where an expense is allowable, it reduces the taxable profit or is claimed through the appropriate route.
Locum doctors working on a self-employed basis may be able to claim expenses directly related to their work, which can include travel between assignments, professional subscriptions, indemnity cover, training and some equipment costs. The key is that costs must be incurred wholly and exclusively for the purposes of the work, and ordinary commuting rules still apply. Records and receipts are essential if an expense is ever queried.
There is no single answer. A limited company can suit some doctors with substantial private or consultancy income, but it brings additional responsibilities such as annual accounts, Corporation Tax returns, payroll and company filing obligations. It can also be unsuitable for certain NHS or locum contract arrangements because of IR35 and off-payroll rules. The right structure depends on your income sources, contracts and longer-term plans, so this is a discussion to have before making a change.
Most medical services provided by registered health professionals are exempt from VAT, which means many doctors do not need to register. However, VAT can become relevant where you provide non-exempt services, sell goods, or where your income exceeds the registration threshold for taxable supplies.
