Not everyone in an associate role technically has to use an accountant, but many find it helpful. Associate dentists are often treated as self-employed for tax purposes, which usually means registering with HMRC, filing a Self Assessment tax return and keeping records of income and allowable expenses. Where you work across several practices, receive a mix of NHS and private income, or have costs such as indemnity cover and GDC registration, an accountant can help make sure everything is recorded consistently and that the figures reported are supported by your records.
The two income streams are usually recorded separately so that each is tracked clearly and matched to the right source documents. NHS income tends to arrive through scheduled payments and statements, while private income may come through fee-per-item charges, plans or treatment packages, and the two can follow different patterns during the year. Keeping them distinct makes it easier to reconcile what you have received, prepare accurate accounts and tax returns, and identify any queries early rather than at year end.
A range of costs may be allowable, depending on your circumstances and how you work. These can include indemnity cover, GDC registration and professional subscriptions, certain continuing professional development costs, specialist equipment and consumables, and a proportion of costs such as home office use or travel between practices where the rules allow. Some items, particularly larger equipment purchases and mixed personal and professional costs, need more careful treatment. The key is that expenses must be genuine business costs, incurred wholly and exclusively for your work, and properly evidenced.
There is no single answer, because the right structure depends on your profits, how many principals or partners are involved, your plans for the practice and how you want to take money out of the business. A sole trader arrangement is simpler to run, while a partnership can suit two or more principals sharing responsibility, and a limited company may offer different tax and liability considerations. The tax position of each option can change as your circumstances change, so it is worth reviewing periodically rather than setting up once and leaving it indefinitely.
It depends on the services provided and the level of taxable turnover. Many clinical dental services are exempt from VAT, but a practice may also supply items or services that are treated differently, which can affect whether registration is required and how VAT is accounted for. Mixed practices therefore need to look at their income in detail rather than assuming a single rule applies. Because the position varies, it is worth reviewing your income streams and any planned changes with an accountant before making a decision.
Yes, payroll support can cover employed dental nurses, reception and administrative teams, hygienists and other staff, including the calculations, payslips and reporting that go with running a payroll. Associates are usually treated differently from employees, so it is important to establish the correct status for each person working in the practice. Where a practice has a mix of employed staff and self-employed associates, the payroll and contractor arrangements should be reviewed together to keep the position consistent.
Switching is usually more straightforward than people expect. In most cases you let your existing accountant know, and we take over the handover of records, prior accounts and tax return information needed to pick up the work. It helps to have your bookkeeping reasonably up to date and to know which returns have already been filed, but where records are incomplete we can work through what is needed. The aim is to make the transition as smooth as possible without disruption to your filing obligations.
It helps to have details of your income sources, whether that is NHS statements, private fee records, associate pay slips or practice drawings, along with records of your business costs and any receipts for larger purchases. If you have previous accounts or tax returns, those are useful for context, as are details of your business structure, VAT status and any employees. If your records are not fully organised, that is not a barrier — part of the initial conversation is working out what is available and what needs to be pulled together.
