A general accountant can prepare accounts and tax returns, but dental work involves arrangements that benefit from familiarity: self-employed associate income from multiple practices, a mix of NHS and private fee income, indemnity and registration costs, practice premises, dental equipment and staff payroll. Working with accountants who regularly deal with dentists can mean fewer basic explanations, more relevant questions about your income and expenses, and advice that takes account of how dental practices actually operate. Associates are commonly treated as self-employed for tax purposes, although this depends on the terms of each arrangement, so the starting point is knowing exactly what you are paid and by whom. We would typically need your income statements or schedules from each practice, records of any tax already deducted, details of expenses you have paid personally such as indemnity cover, professional subscriptions and CPD, plus any other income you receive. From there we can prepare your Self Assessment return, check that the right expenses are being claimed and help you set up a simple bookkeeping routine so nothing is missed at year end.
Both are business income for a self-employed dentist or a practice, but they arrive in different ways and need to be recorded separately so your accounts and returns are accurate. NHS income may come through a contract or schedule of payments, while private income can include treatment fees, plan payments or other charges, and each may raise different record-keeping and VAT questions. The way income is reported also depends on whether you trade as a sole trader, partnership or limited company, so we review your structure before confirming the treatment that applies to you.
There is no single answer that suits every dentist. Sole trader and partnership structures are common and straightforward to administer, while a limited company can sometimes offer different tax and profit-extraction options, though it brings additional compliance and reporting duties. The right choice depends on your profits, how many people are involved, your plans for the practice and your personal circumstances. Expenses that are incurred wholly and exclusively for your dental work are generally allowable, and for dentists these commonly include indemnity insurance, GDC and professional body subscriptions, CPD and training, specialist equipment, clinical clothing, and a proportion of costs such as practice admin or home office use where relevant. Some items, such as equipment, may be treated as capital expenditure rather than routine costs, and costs with a personal element need to be apportioned. Practice payroll can include receptionists, dental nurses, hygienists, therapists, associates and practice managers, and each may be engaged on a different basis, which affects how they are paid and reported.
