In practice that often means tracking NHS contract income separately from private fees, dealing with self-employed associate income from one or more practices, and understanding how practice costs such as indemnity cover, equipment and staff wages should be treated. An accountant who works with dentists regularly will ask about your contract type, your share of fees and your business structure before deciding how anything should be reported.
If you work as an associate and are treated as self-employed, you will usually need to report that income through Self Assessment, keeping records of the fees you receive from each practice as well as the costs you incur in carrying out the work. If you are engaged under an employment contract instead, your tax may be handled through PAYE and your position can be different. Because the answer depends on the terms of your engagement rather than the job title alone, it is worth reviewing your contract before assuming which route applies.
A practice owner typically has additional responsibilities, including payroll for employed team members, employer-related reporting, VAT where registration applies, and the treatment of equipment purchases, premises costs and finance agreements. There may also be questions about how profits are extracted, particularly where the practice trades through a limited company, or how drawings and profit shares are handled in a partnership. An associate's affairs are often simpler, but income from several practices and a mix of NHS and private fees can still make accurate record keeping important.

Mixed income is one of the most common areas where dental accounting needs care, because NHS payments, private treatment fees and other income may need to be tracked separately for VAT and management purposes, even where they are ultimately reported together. Costs incurred wholly and exclusively for your dental work may be allowable, and typical examples include indemnity insurance, professional body and GDC registration fees, professional subscriptions, continuing professional development, and the cost of equipment or materials used in the practice. Whether a particular item can be claimed, and how much of it, depends on the circumstances — capital purchases, items with mixed personal and professional use, and costs already met by an employer all need separate consideration. Not automatically. VAT registration depends on your taxable turnover and on the nature of the services you supply. Many dental treatment services are exempt from VAT, but some cosmetic treatments, laboratory work and sales of goods may be standard-rated, so the position is not always obvious from the turnover figure alone. If you are approaching a registration threshold, or you are already registered and unsure how to treat a particular service, it is worth reviewing the mix of what you supply before making a decision.
Each structure has different implications for tax, national insurance, reporting and the way profits are taken, and there is no single answer that suits every dental business. A sole trader arrangement is often the starting point for a newly self-employed dentist, a partnership may fit two or more principals sharing a practice, and a limited company can be appropriate in certain circumstances. The right choice depends on your income levels, your plans for the practice, how you want to take money out and how much administration you are comfortable with.
