In many cases, yes. If you receive dental income that has not been taxed at source — for example, self-employed associate work, private fees or other payments made without PAYE — that income normally needs to be reported through Self Assessment. The position can be different where all of your income is taxed through PAYE and falls below the level that triggers a filing requirement, so it is worth confirming rather than assuming. An accountant can establish whether a return is required, register you if necessary, and make sure the figures reported reflect your actual income and allowable expenses.
Associate dentists are often paid a percentage of the fees they generate, with the practice retaining a share towards premises, staff and materials. For accounting purposes that usually means recording your gross fee income, tracking the deduction made by the practice, and keeping evidence of both figures. Whether you are treated as self-employed for tax purposes depends on the terms of your contract and how the arrangement operates in practice, so it is worth reviewing your agreement rather than relying on the label used. Getting this right affects what you report and what you may be able to claim.
Keeping NHS contract income, private fees and any plan scheme income clearly separated in your records is generally helpful. Different income streams can involve different reporting and reconciliation requirements, and separating them also makes it easier to see where your profit is actually coming from. Practices holding both an NHS contract and a private list will often need more detailed bookkeeping than those working on a single basis, so the way records are set up matters from the outset.
Broadly, costs that are incurred wholly and exclusively for the purposes of your dental work may be allowable. For dentists these can include indemnity cover, GDC registration, professional body subscriptions, CPD, clinical clothing, laboratory fees and a proportion of certain running costs. Capital items such as equipment and practice fixtures are treated differently and may attract capital allowances instead of a straight deduction. What is allowable depends on your circumstances, so keeping receipts and discussing anything unusual with your accountant is sensible.
There is no single structure that suits every dentist. A sole practitioner with modest profits may find a straightforward arrangement works well, while a practice with several principals, employed staff and reinvested profits may see advantages in operating through a limited company or a formal partnership. Tax treatment, National Insurance, profit extraction, administrative obligations and the practicalities of buying into or leaving a practice all feed into the decision. It is worth reviewing your structure periodically, because what fitted your circumstances at the start may not fit them now.
