A doctor whose entire income is taxed through PAYE, with no untaxed earnings, no significant expenses to claim and no pension complications, may manage perfectly well without one. The picture changes as soon as you take on locum shifts, private practice sessions, consultancy or medico-legal work, partnership profits, a limited company or rental income — at that point you have Self Assessment obligations, expense claims to substantiate and often a choice of business structure to make. An accountant helps ensure the tax position is correct, that you are claiming what you are entitled to, and that deadlines are met without last-minute stress.
Common examples include medical indemnity insurance, professional body subscriptions, costs of training and professional development related to your current work, equipment and reference materials, and travel or mileage for work-related journeys. What is allowable depends on whether you are employed, self-employed, or both, and on the precise purpose of the expense — for instance, travel between sites can be treated differently from ordinary commuting, and training costs have their own rules. Because the detail matters, it is worth reviewing your spending patterns with an accountant rather than assuming a blanket list applies to you.
NHS pay is normally taxed through PAYE, while private fees, locum work and consultancy income are usually reported through Self Assessment. Operating as a sole trader is generally simpler, with Self Assessment and Class 2 and 4 National Insurance to consider, while a limited company can offer different tax treatment but brings company accounts, Corporation Tax, payroll and often VAT into play. There is no universal answer, and the decision can change as your earnings grow or your work shifts between locum, private and salaried roles — so it is worth reviewing properly rather than choosing on the basis of what someone else does.

Partnership work typically involves partnership accounts, allocation of profits between partners, and support with the wider financial reporting and record keeping the practice needs to maintain. Partners also have individual Self Assessment obligations, and their profit share interacts with pension contributions and other income. If you are a partner, the useful starting point is a review of the current accounts, how profits are allocated and whether the reporting arrangements are working well for everyone involved.
