It depends on your circumstances. A doctor with a single salaried NHS post taxed entirely through PAYE may have fairly simple affairs, although a return can still be required for reasons such as additional income or pension-related tax charges. Doctors with locum work, private practice, consultancy income, partnership or company involvement, property income or investments usually benefit significantly from professional help, because several income sources need to be brought together accurately on one return. Even where the filing itself is simple, advice on allowable expenses, pension contributions and structure often pays for itself in peace of mind.
Not every doctor does, but many do. If all of your income is taxed through PAYE and there is nothing else to report, a return may not be necessary. A return is more likely to be required where you have locum or private income, self-employment or partnership income, untaxed savings or investment income, or certain pension-related tax charges to report. Because HMRC expects you to know whether a return is due, it is worth checking your position rather than assuming PAYE has covered everything.
Commonly claimed items include medical indemnity cover, professional body and registration subscriptions, relevant training and continuing professional development, specialist equipment and business travel or mileage. However, the rules differ depending on whether you are employed or self-employed. Employees can generally only claim expenses that are wholly, exclusively and necessarily incurred in performing their duties, while self-employed doctors may have wider scope. Costs with a dual purpose, such as general clothing or everyday commuting, are not allowable, so it is worth reviewing your spending before assuming it can be claimed.
Yes. Locum work often means several pay streams across a tax year, sometimes through agencies and sometimes paid directly, alongside any salaried or private income. We bring these together so that everything taxable is declared, expenses are claimed correctly and your records support the figures in your return. If you work through an agency, we can also look at how the engagement terms affect your tax treatment and whether the structure you are using remains appropriate.
There is no single right answer. A sole trader structure is straightforward and often suits doctors with modest private or locum income, while a limited company can be worth considering where profits are retained or where engagement terms require it. Partnerships are common where doctors share a practice. Tax treatment, administrative obligations, pension implications and the nature of your contracts all influence the decision, and the right structure for you can change as your work develops.
Most medical care provided by registered health professionals is exempt from VAT, which means no VAT is charged on those fees and VAT on related costs is generally not recoverable. However, some services fall outside that exemption, and the position can be less clear for mixed private practices offering non-medical or cosmetic treatments. VAT registration depends on the level of taxable turnover, so if part of your work is not exempt it is worth reviewing the position rather than assuming all medical income is treated the same way.
