It depends on your circumstances. A doctor with a single NHS salary taxed entirely through PAYE may not need much ongoing support, although a review can still be useful if you have additional income, claim professional expenses or want to check your pension position. Where you have locum income, private fees, consultancy work, a partnership or a limited company, an accountant can help ensure returns are accurate, expenses are claimed correctly and deadlines are met. The value is usually greatest when income comes from more than one source.
Many doctors do, particularly if you receive untaxed income such as locum fees, private consultation income or consultancy payments, or if you have income from a partnership or a limited company. Higher and additional rate taxpayers with certain income types may also need to file. If your only income is a salary taxed under PAYE and it is fully covered by your tax code, a return may not be required — but the position depends on your individual circumstances and is worth checking rather than assuming.
Locum work is often treated as self-employment, which means you report your income and allowable expenses on a Self Assessment return and pay income tax and National Insurance through the self-assessment system. Some locum arrangements are taxed through PAYE instead, and the correct treatment depends on how the engagement is structured. Because the tax and National Insurance consequences differ, it is worth reviewing your contract and working arrangements rather than treating every locum shift in the same way.
There are several expenses doctors commonly claim where they relate directly to their work, including medical indemnity insurance, GMC and royal college or professional body subscriptions, specialist equipment, and training or CPD costs in certain circumstances. Business travel and mileage may also be claimable, but ordinary commuting generally is not. Whether an expense is allowable depends on the nature of your work, your employment or self-employment status and the specific rules that apply, so claims should be reviewed rather than assumed.
It can be worth considering, but it is not automatically the right answer for everyone. A limited company may suit some doctors with substantial private or consultancy income, while sole trader or partnership structures can be more appropriate for others. The decision affects how tax and National Insurance are paid, how accounts and Corporation Tax are handled, how profits are extracted and how much administration is involved. A review of your income levels and working arrangements is the sensible starting point.
We look at both together. NHS pay is usually taxed through PAYE, while private fees, locum income and consultancy work often need to be reported separately, so the key is making sure the correct figures appear on the right returns and that professional expenses are allocated sensibly. Where private work is run through a practice or a limited company, the accounting and reporting requirements differ again. Treating each income stream in isolation tends to create errors and missed reliefs.
