It depends on your circumstances. If you are a salaried doctor with a single NHS employer and straightforward tax affairs, your position may be relatively simple, though you will still want to be confident that medical indemnity, professional subscriptions and other allowable costs are being claimed correctly. Where an accountant tends to add most value is when your income comes from more than one source, for example NHS pay plus private fees, locum shifts, consultancy or medicolegal work. Multiple income streams increase the chance of errors, missed expenses and unexpected tax bills, and professional support helps keep all of it accurate and organised.
Your NHS salary is normally taxed through PAYE, but private fees, locum payments and consultancy income usually need to be reported on a Self Assessment tax return alongside it. The return brings the different sources together so HMRC can calculate the correct overall liability. In practice, this means keeping clear records of private income and the costs associated with earning it, and being aware that once your total income rises you may be asked to make payments on account towards the following year. The precise treatment depends on your individual position, which is why we review it rather than applying a standard approach.
Commonly claimed costs include medical indemnity insurance, professional body subscriptions such as GMC and royal college fees, and certain training or professional development costs where they are relevant to your existing work. Equipment, professional books and travel to temporary or multiple work locations may also be allowable in the right circumstances. What matters is that the cost is incurred wholly and exclusively for the purposes of your work and that it is claimed against the correct income source. Some costs are clearly allowable, others sit in greyer territory, and we assess each one against the rules rather than assuming.
Locum income is generally treated as self-employed earnings if you are engaged on that basis, rather than as employment income, which means it is reported on a Self Assessment return and you are responsible for the associated tax and National Insurance. You will usually want to keep records of each engagement, the income received, and expenses such as travel and indemnity cover. Where locum work sits alongside a salaried NHS post or private practice, the combination needs to be reported carefully. The engagement terms matter, because they influence how the income is classified.
There is no single answer, because it depends on your level of private income, how you want to extract profits, your pension position and how much administrative work you are willing to take on. A limited company can suit some doctors with substantial private or consultancy earnings, while a sole trader or partnership arrangement may be simpler and more appropriate for others. IR35 rules also need to be considered where contracts are involved. We talk through the practical and tax implications of each option so you can make an informed decision rather than following a general rule of thumb.
Yes, where those obligations apply. Many private medical services are exempt from VAT, but not all activities are treated the same way, and medicolegal or consultancy work can sit outside the exemption. If you employ reception, administrative or clinical staff, payroll needs to be run correctly with the appropriate deductions reported to HMRC. We assess whether VAT registration is required or advisable in your case and, where you are registered, handle the reporting. Payroll, bookkeeping and management accounts are often handled alongside your annual accounts.
Many doctors find remote accounting support convenient because it fits around clinical rotas and unpredictable working patterns. Records can be shared securely online, meetings can be held by video or phone, and there is no need for your accounting arrangements to depend on when you happen to be free during office hours.
