Many doctors do, particularly once income comes from more than one source. If you are purely salaried with no private fees, locum work or untaxed income, your tax may be handled through PAYE and your position could be simple. But as soon as you take on private patients, locum shifts, consultancy, medico-legal reports or partnership profits, you are likely to need to file a Self Assessment return, and the calculations around expenses, pension contributions and tax payments become more involved. An accountant can help make sure everything is reported correctly and that reliefs you are entitled to are not missed.
Your NHS salary is taxed through PAYE, while private fees, locum income and most consultancy work are reported through Self Assessment. The two are not taxed separately — your total income across both is brought together to determine your overall tax position, which can mean additional tax is payable on the self-employed element. Because the interaction affects how much you pay and when, it is worth planning payments on account rather than waiting for a return to be filed.
It depends on the type of income the expense relates to and your working arrangements. Costs that are incurred wholly and exclusively for self-employed work — such as medical indemnity cover, professional subscriptions, certain training and professional development costs, equipment, and business mileage — may be claimable. The position for employed income is more restrictive, and some items are only allowable in specific circumstances or where they appear on HMRC's approved list. Because the rules differ, we review expenses individually rather than applying a blanket list.
Medical indemnity and defence union subscriptions are generally treated as allowable where they relate to self-employed income, such as private practice or locum work. For employed doctors the position is more limited, and whether a subscription can be claimed depends on the nature of the cover and the relevant rules. If you have a mix of NHS and private work, the same policy may need to be looked at in more than one way, so it is worth checking before assuming the full cost is deductible.
Not automatically. Many locum doctors work as sole traders, which is straightforward to administer, while others find a limited company suits higher or more varied earnings. The decision depends on factors such as your expected profits, how your engagements are contracted, and whether off-payroll working rules apply — for example, where services are provided through an intermediary to an NHS body or another public sector client. We look at your particular circumstances rather than recommending incorporation as a default.
It depends on the nature and level of your income. Many healthcare services provided by registered medical professionals are exempt from VAT, so a purely clinical private practice may not need to register. However, not all medical-related work is treated the same way, and advisory, report-writing or other non-clinical services can fall outside the exemption. If your income is mixed, or you are unsure how your services are classified, a review of your position is usually the sensible first step.
NHS pension contributions are handled through the scheme and your payroll, but they still matter for your wider tax position — particularly where you also have private income or make additional pension contributions. Annual allowance questions can arise for doctors with high or fluctuating earnings, and the calculations are not always intuitive. This is an area where a review well before the tax return is due tends to be more useful than a retrospective fix.

What matters is that your records are complete and your filing dates are met, not that documents change hands in person. We agree a working method that suits your schedule from the outset.
