Not every doctor does. If your income is entirely salaried and taxed through PAYE, and you have no untaxed income, you may be able to manage without one. But once private fees, locum income, consultancy work, a partnership share or a limited company enter the picture, the position becomes more involved. An accountant can help you understand what has to be reported, keep records that stand up to scrutiny, and make sure you are claiming the professional expenses you are entitled to. Many doctors also value the reassurance of knowing their affairs are handled properly while they concentrate on clinical work.
If all your income is taxed through PAYE and there is no untaxed income to declare, you may not need to file. However, Self Assessment can still be relevant for doctors with higher incomes, additional income such as savings, dividends or rental income, or those who need to report pension-related tax charges. It is also worth checking whether your PAYE code is collecting the right amount, particularly if you have changed roles, sessions or employers during the year. We can review your position and confirm whether a return is required.
Your NHS salary is normally taxed through PAYE as it is paid, while private fees are often received gross and only taxed once reported. That means the tax on private income can arrive as a balancing payment, followed by payments on account towards the following year. Setting aside an appropriate amount as income comes in, and reviewing your position partway through the year, usually avoids any nasty surprises. The exact treatment depends on how the income is structured and whether it is paid to you personally or to a company.
Common examples include medical indemnity cover, GMC registration, royal college and specialist society subscriptions, professional journals, CPD and training costs, specialist equipment and professional travel including mileage between work locations. What can actually be claimed depends on whether you are employed or self-employed, since the rules for employees are stricter and require costs to be incurred wholly, exclusively and necessarily in performing your duties. Records matter as much as the claim itself, so keeping receipts, invoices and a mileage log makes the process far smoother.
It depends on how you are engaged. Some locum work is paid through PAYE, either by the trust or an agency, while other arrangements are genuinely self-employed. Getting that distinction right matters, because it affects whether you need to register with HMRC, what you can claim as expenses, how National Insurance is handled and whether a Self Assessment return is required. Locums often work across several sites and trusts, so keeping clear records of each engagement, the income received and the travel involved is particularly important.
It depends on your circumstances. A limited company can suit doctors with substantial private or consultancy income, offering a different approach to how profits are drawn, but it brings additional obligations including company accounts, Corporation Tax, payroll and confirmation statements. It is also not appropriate in every situation, and some NHS or public sector engagements come with rules that affect how you can be engaged. We look at your level of income, the nature of the work and your longer-term plans before recommending a structure.
VAT on medical services is not straightforward. Depending on your professional registration and the exact nature of the service, some clinical work can be exempt, while activities such as medico-legal reports, cosmetic or non-medical services, and certain consultancy arrangements may be standard-rated. VAT registration is based on taxable turnover, and the threshold is subject to change, so it is worth reviewing your position as private income grows. We can assess which parts of your work are taxable and what that means for registration and pricing.
Changing accountants is usually straightforward. Once you decide to move, we contact your existing accountant to request professional clearance and the handover of your records, then review your position and pick up any outstanding filings. In the meantime, we ask you to share details of your income sources, business structures and any current obligations so we can see the full picture. If you are approaching a filing deadline, it helps to start the process early rather than waiting until the last moment.
