Not every doctor does. If you are employed on a single NHS payroll, pay tax through PAYE and have no other income, your tax affairs may be manageable without professional help. An accountant becomes far more valuable once your circumstances include private fees, locum income, consultancy or medico-legal work, a practice or partnership interest, or a mix of employment and self-employment. At that point, the questions tend to be about what is allowable, how income should be reported, what structure suits you and when tax is actually payable. Even doctors with straightforward income sometimes use an accountant to review expenses and confirm their position.
Your NHS salary is taxed through PAYE, and your private income is usually reported through Self Assessment. The return brings both together so HMRC can calculate the correct overall liability, which may mean additional tax is due on the private earnings, and potentially on the NHS income if your total pushes you into a higher band. If tax is owed, payments on account towards the following year are often required, so the amounts and dates can differ from what you might expect. Accurate records of private fees and related expenses make the whole process far more straightforward.
Common claims include medical indemnity insurance, professional body subscriptions and registration fees, and the cost of training or equipment where it is relevant to your work. Whether a cost is deductible depends on the rules that apply to your situation, which differ between employment and self-employment, and on whether the expense was incurred wholly and exclusively for the purposes of the work. Mileage and travel between sites can also be claimed in the right circumstances, though ordinary commuting generally cannot. Because the detail matters, it is worth reviewing your expenses rather than assuming a fixed list applies to everyone.
Locum work is often treated as self-employment, particularly where you arrange your own shifts and work across different practices or trusts, although arrangements vary and some locum roles are employed positions. If your locum income is self-employed, you will usually need to register with HMRC, keep records of income and expenses, and report the profits through Self Assessment. You may also need to budget for payments on account and, in some cases, consider whether working through an intermediary affects your position. It is worth checking how each engagement is structured, because the tax treatment follows the facts.
It depends on your circumstances. A limited company can offer a different approach to how profits are extracted and may suit doctors with substantial private income, consultancy work or medico-legal earnings, particularly where profits are retained in the business. It also brings additional obligations, including annual accounts, Corporation Tax returns and payroll for directors. For doctors whose private income is modest, or whose work is largely NHS-based, sole trader or partnership status may be simpler and just as appropriate. The right answer depends on your income mix, plans and how much administrative complexity you want.
Yes. Partnership accounting typically involves preparing the practice accounts, allocating profits between partners in line with the partnership agreement, and using that information to complete each partner's Self Assessment return. Practice owners may also need payroll for staff, VAT advice, management information and support with capital expenditure and drawings. Because partnership profit shares and the underlying agreement do not always match up neatly, it helps to review the figures each year and flag anything that needs attention.
They can. Pension contributions may attract tax relief, and NHS pension arrangements have their own rules and thresholds that can create additional tax charges for higher earners. Private pension contributions are treated differently again and may affect the amount you can contribute overall. Because pension rules interact with your total income and can change, this is an area where it is worth looking at your position as a whole rather than in isolation, and reviewing it as your earnings change.
You do not need to be based close to us to work together. Documents can be shared securely and meetings held by phone or video, which suits doctors with busy rotas. If you are moving from another accountant, we can usually handle the handover by contacting your existing firm for the relevant records and confirming your position with HMRC, and you simply need to let them know you are moving. It is helpful to have your last set of accounts or tax return, your HMRC references and details of your income sources to hand when we begin.
