Not every doctor does. If your only income is an NHS salary taxed entirely through PAYE and you have no private income, savings complexity or untaxed earnings, your affairs may be straightforward. The picture usually changes once you take on private fees, locum work, consultancy, medico-legal reports or partnership profit, because you then need to report income through Self Assessment and claim expenses correctly. Doctors also often have pension contributions, indemnity costs and professional subscriptions to account for, which is where specialist help tends to be worthwhile.
Your NHS salary is normally taxed through PAYE, and your private or locum income is reported on a Self Assessment tax return alongside it. The return brings together your pay records, private income, pension contributions and allowable expenses, and HMRC then calculates any additional tax due after accounting for what has already been paid. Issues often arise where tax codes do not reflect private income, where expenses are claimed inconsistently, or where payments on account are misunderstood. Getting the figures right and consistent year to year is the priority.
How you are engaged matters. Some locum work is paid through PAYE, some through an agency or umbrella arrangement, and some directly to a limited company. You should keep a record of every engagement, the payer, gross and net amounts, any tax deducted, and the expenses you incurred, including travel between sites, indemnity cover and professional subscriptions. If you work through an agency in the public sector, off-payroll working rules may affect how you are taxed, so it is worth reviewing your arrangement rather than assuming last year's approach still applies.
Common claims include medical indemnity insurance, GMC and royal college or specialist society subscriptions, job-related training and continuing professional development, professional books and journals, specialist equipment, and business mileage where you travel between work locations. Each claim needs to be genuinely incurred in carrying out your work and supported by evidence. What is allowable depends on whether you are employed or self-employed and on the specific nature of the cost, so it is worth checking before assuming something qualifies.
Medical indemnity cover is generally treated as an allowable professional expense where it is required for your work, whether you are employed or self-employed, although the way relief is given can differ depending on how you are taxed. Because the treatment can vary, and because indemnity arrangements differ between doctors, it is best to review your own policy and circumstances rather than rely on a general assumption.
It depends on the level and regularity of your private income, how your contracts are structured and your wider financial position. A limited company can offer flexibility over how profits are extracted and may be administratively sensible for some doctors, but it brings additional filing, payroll and accountancy obligations and may not suit those with smaller or occasional earnings. Off-payroll working rules can also affect whether a company structure is appropriate for particular engagements. It is a decision best made after reviewing your actual circumstances.
Much of the medical care doctors provide is exempt from VAT, which means many doctors never need to register. However, not all services fall within the exemption. Medico-legal reporting, certain non-medical services and some cosmetic or aesthetic work may be treated differently. If you provide services of that kind, or your turnover from taxable work approaches the registration threshold, you should review your position. Registration obligations depend on turnover and on the nature of the services, so the answer is specific to what you actually do.
