Not every doctor does. If all your income is taxed through PAYE, your tax affairs may be simple enough to manage without professional help. However, once you take on private work, locum shifts, consultancy income, medicolegal work, a partnership share or a limited company, the picture becomes more involved. At that point an accountant can help you report income accurately, claim the expenses you are entitled to, and avoid the penalties and interest that can follow an incorrect or late return. Doctors with more complex arrangements often find the reassurance and time saved are worthwhile.
It depends on where your income comes from. If you are employed and all your pay is taxed through PAYE with no other income, you may not need to file. If you receive private fees, locum payments, consultancy income, untaxed savings or rental income, or if HMRC has asked you to submit a return, you will usually need to file one. Doctors who hold a company directorship, sit on a partnership, or have income that is not taxed at source should generally expect to complete a return each year.
NHS employment income is normally taxed through PAYE, with tax deducted from your salary and reported through your payslips. Private fees, locum earnings and consultancy income are usually paid gross, meaning tax is not deducted at source, so they generally need to be reported and taxed through Self Assessment. Because both types of income can affect your overall tax position, and because expenses and pension contributions may apply differently, it is worth having the figures reviewed together rather than in isolation.

Medical indemnity or defence union cover, GMC registration fees and subscriptions to relevant professional bodies such as royal colleges are commonly claimed, provided they relate to your professional work. Employees can generally claim these where they are required for the job, and self-employed doctors may treat them as business expenses. The exact treatment depends on your employment status and the nature of the payment, so it is worth checking before submitting your return.
Locum work usually generates self-employed income, which means you will typically need to register with HMRC, keep records of every placement and payment, and retain receipts for related costs. Common claims include travel and mileage between placements, medical indemnity, professional subscriptions, equipment and training. Keeping a running record of shifts, income and expenses throughout the year makes your return far easier to prepare and reduces the risk of understating or overstating your profits.
There is no single answer that suits everyone. Working through a limited company can be appropriate for some private practitioners, consultants and medicolegal practitioners, but it brings additional obligations such as statutory accounts, Corporation Tax returns, and company payroll and dividend considerations. It also may not be suitable where IR35 rules or NHS pension implications apply. Whether a limited company is beneficial depends on your income level, contract terms and long-term plans, so structure should be reviewed against your own circumstances rather than copied from someone else.
