It depends on your circumstances. If you are employed on a straightforward NHS contract with no other income, your tax affairs may be simple. But once you add private medical income, locum shifts, consultancy work, a partnership share or a limited company, the position becomes considerably more involved. An accountant can help ensure the right expenses are claimed, that Self Assessment is accurate, and that your structure still works for how you are actually earning. Many doctors find the reassurance of having the position reviewed properly is worth more than the fee itself.
Your NHS salary is taxed through PAYE, while private medical or locum income is usually reported through Self Assessment. Both are brought together on your tax return, and tax already paid through PAYE is offset against your overall liability. Two things commonly catch doctors out: the interaction between PAYE and your tax code, and payments on account, which can create a larger bill than expected in the early years of self-employment. Getting the figures right at the outset avoids most of the difficulty.
Medical indemnity cover, professional body subscriptions and similar costs are often allowable, but the precise treatment depends on whether the expense relates to employed or self-employed work and whether it meets the relevant rules. For employed doctors, relief is generally available for professional fees and subscriptions on the approved list. For self-employed doctors, the test is whether the cost was incurred wholly and exclusively for the purposes of the trade. We review these costs as part of your return rather than assuming either way.
Locum work can be engaged in several ways — directly with a practice or trust, through an agency, or via your own limited company — and each has different tax and administrative consequences. Sole trader status is common and relatively simple, but a limited company can offer different planning options depending on your income and plans. Where you provide services through a company to a public sector body, the off-payroll working rules may also need to be considered. The right structure depends on your individual circumstances, so it is worth talking it through before committing.
Not automatically. A limited company can bring advantages around profit extraction, pension contributions and retained earnings, but it also means company accounts, Corporation Tax returns, payroll and additional compliance obligations. For doctors with lower levels of private income, sole trader or partnership status is often perfectly adequate. The decision should be based on your income levels, how you intend to draw money from the business, and how much administrative work you are comfortable with.
For self-employed or private practice income, you should keep records of invoices issued, fees received, bank statements, expense receipts, mileage logs and any payroll or pension information. HMRC generally expects self-employment records to be retained for at least five years after the relevant filing deadline, and longer in some circumstances. Keeping these organised through the year, rather than reconstructing them afterwards, makes both your tax return and any enquiry far less stressful.
Both approaches work. Many doctors prefer to handle things remotely because rotas, shift patterns and travel make fixed appointments difficult, and documents can be shared securely online. Others prefer a mix of remote communication and occasional conversations. What matters is that you have a clear point of contact and that your records reach us in a workable format, whether that is digital bookkeeping, scanned documents or something else.
