It depends on your circumstances. If your only income is a straightforward NHS salary taxed through PAYE and you have no untaxed income, you may not need one. But most doctors have something that complicates matters — locum shifts, private consultations, medico-legal work, a partnership profit share, rental income or higher-rate tax relief on pension contributions. Once Self Assessment is involved, or once you are weighing up expenses and structures, specialist help tends to save more than it costs in both time and tax efficiency.
Your NHS salary is taxed through PAYE, but private and locum earnings are usually reported through Self Assessment, where all your income is brought together. Tax already paid through PAYE is credited against your overall liability, and any shortfall is payable by the filing deadline, often alongside payments on account towards the following year. Because the combined figures can push you into a higher band or reduce your personal allowance, planning ahead matters far more than it does for doctors on a single salary.
Common claims include medical indemnity and defence union subscriptions, GMC and royal college fees, specialist society memberships, required CPD and training, professional books and journals, equipment used for your work, and business mileage. What is allowable depends on whether the cost relates to employed or self-employed work, and on whether it is wholly and exclusively for that work. Some costs are claimed through Self Assessment, others through PAYE if your employer operates the relevant scheme, so the route differs case by case.
Locum work is often treated as self-employment, which means registering with HMRC, keeping records of income and expenses, and reporting through Self Assessment. Where you are engaged through an agency, the position can be different and may depend on the contract terms and off-payroll working rules. Because locum earnings are irregular, we usually look at payments on account and set aside arrangements at the same time, so the tax bill does not arrive as a surprise.
Yes. Some doctors operate through a limited company for private, consultancy or medico-legal work, while others work in partnership, particularly in general practice. The right structure depends on your income levels, contract terms, IR35 position and personal plans, and it is not a decision that suits every doctor. We can review what you currently have, explain the corporation tax, dividend, National Insurance and administration implications, and advise on whether a change is worth making.
VAT registration depends on the level and type of your income rather than your job title. NHS income is generally outside the scope of VAT, but private practice, consultancy and some medico-legal work may be taxable supplies, and once turnover from those activities passes the registration threshold, registration usually becomes compulsory. Some healthcare services are exempt, so the analysis needs to look at exactly what you provide. We can review your position and advise on registration, record keeping and returns.
At a minimum, keep payslips and P60s, self-employment invoices and receipts, bank statements for any business or practice account, mileage records, pension contribution statements, and paperwork for indemnity cover, subscriptions and training. If you run a practice or company, you will also need payroll records, supplier invoices and details of any assets purchased. Good record keeping makes returns quicker to prepare, reduces the risk of missed claims and gives you a clearer view of your actual earnings.
