It depends on your circumstances. If you are solely salaried through PAYE and have no other income, your tax is largely handled by your employer and you may not need much support. However, once you add private practice fees, locum shifts, consultancy work, rental income, significant investment income or higher-rate pension contributions, your affairs usually become complex enough that professional help saves time and reduces the risk of errors. Doctors who are self-employed, in partnership or running a limited company generally need accounts and returns prepared as a matter of course.
NHS salary is taxed through PAYE, so it is not usually taxed again on your return, but it still needs to be reported because it affects your overall tax position. Private medical income is generally self-employment income, which means you report income and allowable expenses and pay tax and Class 4 National Insurance through Self Assessment. The combination can push you into higher tax bands or affect your payments on account, so it is worth reviewing how the two are brought together rather than treating them as separate exercises.
Commonly claimed items include medical indemnity insurance, GMC or specialist registration fees, professional body subscriptions, and certain training or continuing professional development costs that maintain or update skills in your existing field. Specialist equipment, professional books and journals, and business travel or mileage may also qualify where they are genuinely used for your work. Purely personal costs, ordinary commuting to a permanent workplace, and general clothing are not allowable. Each claim depends on your circumstances and needs to be supported by records.
Locum work is normally treated as self-employment, which means keeping records of invoices, expenses and mileage, registering for Self Assessment if you have not already, and budgeting for payments on account. Depending on your turnover and whether you work through an agency or directly with practices, VAT registration and possibly a limited company structure may become relevant. An accountant can help you set up a straightforward bookkeeping routine, prepare your return accurately and plan for tax payments rather than being surprised by them.
There is no universal answer. Sole trader status is simpler and cheaper to administer, while a limited company can offer different tax and National Insurance outcomes and may suit doctors with substantial private or consultancy income, particularly where profits are retained or shared. The right choice depends on your income levels, whether you need flexibility over drawings, how much administration you want to take on, and your longer-term plans. It is worth reviewing periodically, as the answer can change as your work develops.

Medical indemnity cover, GMC registration and relevant professional body subscriptions are among the expenses doctors most commonly claim, and they should be recorded with supporting documentation such as renewal notices and receipts. The key is evidencing that the cost relates to your professional work. We can review your expenditure and identify which items are likely to be allowable, and which are better treated as personal or non-deductible.
In a partnership, the practice prepares accounts and the profit is allocated between partners according to the partnership agreement. Each partner then reports their share on their own Self Assessment return, with tax and National Insurance calculated on their individual position. This means partnership accounts, profit shares, drawings, and any practice-level VAT or payroll matters all need to be coordinated. Clear communication between the practice and each partner's accountant helps ensure the figures reported are consistent.
Remote working is well established, and documents can be shared securely online, which suits doctors with busy or irregular schedules. If you already have an accountant, moving across is usually straightforward: we contact your existing accountant for professional clearance and the relevant records, review your previous returns and accounts, and confirm your filing position before taking over. It helps to make the change outside a busy filing period where possible, but it can be managed at most times of the year.
