Not every doctor does. If you are employed on PAYE with no other income, your tax is usually handled through your salary, although you may still benefit from advice on pension contributions, expenses or planning. An accountant becomes considerably more valuable once you have income that is not taxed at source, such as locum fees, private practice income, consultancy work or medico-legal fees. In those situations you will usually need to register for Self Assessment, declare all your income correctly and claim the expenses you are entitled to, and getting that wrong can lead to interest or penalties.
Your NHS salary is normally taxed through PAYE, and your private or locum income is generally reported through Self Assessment, where it is taxed alongside your employed income. You may also be asked to make payments on account towards the following year's tax bill, depending on your liability. The practical challenge is keeping accurate records of both streams and making sure expenses are allocated to the right source, since the rules differ between employed and self-employed work. We handle that reconciliation as part of preparing your return.
It depends on whether the cost relates to employed or self-employed work. For self-employed doctors and locum work, medical indemnity insurance, professional body subscriptions, revalidation and CPD costs, specialist equipment, reference materials, administrative support and business travel can often be claimed, provided they are wholly and exclusively for the purpose of the work. For employed doctors, the rules are tighter, and typically only costs that are necessary for the job and not reimbursed by the employer qualify. We review your actual outgoings rather than applying a blanket list.
Locum doctors often travel between multiple sites, and the tax treatment of that travel depends on the pattern of your work rather than simply the distance covered. Journeys between separate locum engagements or to temporary workplaces can sometimes be treated as allowable business travel, while ordinary commuting is not. Keeping a clear mileage log with dates, destinations and purposes gives you a defensible record and makes the year-end process far easier. We can help you set up a simple system for capturing this throughout the year.
There is no single right answer. Sole trader status is simpler and involves less administration, while a limited company can offer different tax treatment and limited liability in some circumstances. The most suitable option depends on your level of private or locum income, whether you have partners or associates, your appetite for administration, and your longer-term plans. It is also worth revisiting as your income mix changes. We can talk through the practical implications of each structure before you commit.
Many medical services are exempt from VAT, but that does not automatically mean VAT is irrelevant to your practice. The position depends on the specific services you provide, whether you supply any goods, and whether you have non-medical income streams. If you are registered or required to register, Making Tax Digital rules apply to your record keeping and returns. We can assess whether VAT applies to your activities and, if so, manage the registration and filing obligations for you.

It is usually more straightforward than people expect. We start by understanding your current position, including your income sources and existing records, and then we contact your previous accountant for the relevant handover information and professional clearance. Once we have your records and any outstanding filings are up to date, we agree how the ongoing relationship will work. If there is anything urgent, such as an approaching filing deadline, we will flag it early so nothing is missed during the transition.
