If you are employed on a straightforward salary with no other income, PAYE usually handles your tax, though you may still benefit from checking that your professional expenses and any pension-related position are correct. Once you take on locum work, private fees, consultancy income or a partnership share, you become responsible for reporting that income through Self Assessment, and the calculations become more involved. An accountant helps ensure all income sources are reported correctly, that allowable expenses are claimed and that you are prepared for the tax payments that follow.
Your NHS salary is normally taxed through PAYE, while private fees, locum payments and consultancy income are often paid without tax deducted. VAT registration depends on your taxable turnover and the type of services you provide, and there are exemptions that can apply to some medical services. Whether a limited company is appropriate depends on your income level, how your work is structured, your pension position and your wider plans. These are decisions with long-term consequences, so they are best made after reviewing your specific circumstances rather than following a general rule.
