It depends on your circumstances. If you are employed solely through PAYE with no other income, no significant expenses and straightforward tax affairs, an accountant may not be essential. But most doctors have some complexity, whether that is private fees, locum income, consultancy work, a partnership profit share, a limited company or simply a set of professional expenses worth claiming correctly. Where income comes from more than one source, or where Self Assessment is required, professional support usually pays for itself by keeping filings accurate and by identifying planning points that are easy to overlook.
Yes. Combining employed NHS income with private fees, locum earnings or consultancy income is one of the most common situations we deal with for doctors. The two income types are reported differently on a tax return, and the interaction with tax already deducted through PAYE needs to be worked through carefully. We gather the relevant figures from your payslips, P60, private income records and expense documentation, prepare the return, and explain the resulting position, including any payments on account that may follow. Because everyone's mix of income differs, the outcome is always specific to your own situation.
Commonly claimed items include medical indemnity insurance, professional body subscriptions and registration fees, certain training and professional development costs, professional equipment, and business travel or mileage where journeys qualify. As a rule, an expense generally needs to be incurred wholly and exclusively for the purposes of your professional activity, which means ordinary commuting and purely personal costs do not qualify. The distinction between allowable and non-allowable costs can be subtle, particularly where you work at several sites or combine employed and self-employed work, so it is worth reviewing your spending with an accountant before assuming what can be claimed.
Locum work is usually treated as self-employment for tax purposes, which means you need to register for Self Assessment, keep records of income and expenses, and file a return each year. Many locums also work through a limited company, which can change both the tax treatment and the administrative obligations considerably. Your position depends on how you are engaged, how much you earn, your other income and whether you operate as an individual or a company. We look at your particular arrangements rather than applying a general rule, and can advise on whether your current structure remains appropriate as your work changes.
There is no universal answer, because it depends on how much you earn, how you take money out of the business, your pension position and your wider financial plans. A limited company can offer a different tax treatment and a clearer separation between personal and professional finances, but it also brings additional compliance obligations, including annual accounts, Corporation Tax returns and potential payroll requirements. For some doctors the benefits are clear, for others they are marginal or outweighed by extra administration. We work through the numbers and the practical implications with you before you commit to a structure.
Partnership accounts for a medical practice involve allocating profits between partners according to the partnership agreement, tracking drawings and capital accounts, and preparing the partnership tax return alongside each partner's individual Self Assessment. Payroll for practice staff and pension arrangements often sit within the same picture. Accuracy matters here because each partner's tax position depends on the allocation being correct, and discrepancies can be costly to unravel. We coordinate the practice side and the individual side together so the figures are consistent.
