It depends on your circumstances. A doctor whose entire income is taxed through PAYE with no additional income sources may have little to report beyond routine matters. But once private fees, locum income, consultancy work, rental income or untaxed earnings are involved, a Self Assessment return is usually required, and the return has to account for every source correctly. An accountant's role is to make sure nothing is missed, that claims are properly supported, and that you are not paying more tax than you need to — as well as dealing with HMRC on your behalf if questions arise.
Your NHS pay is normally taxed through PAYE, with tax deducted before you receive it. Private fees, locum earnings and consultancy income are often paid gross, meaning no tax has been deducted at source. Self Assessment brings both together: the return reports your total income, gives credit for the tax already paid through PAYE, and calculates any further liability. This is why keeping accurate records of private income and associated expenses matters — the return needs to be complete, not approximate.
Commonly considered costs include medical indemnity and defence organisation subscriptions, professional registration and body membership fees, training and continuing professional development that maintains your existing expertise, specialist equipment and reference materials, and travel to temporary or multiple workplaces. What is allowable depends on whether the cost relates to employed or self-employed work, whether it is incurred wholly and exclusively for that work, and whether the rules on travel and equipment are met. We review your expenditure and confirm what can be claimed and what should be excluded.
It often does. Locum work is frequently treated as self-employment, which means income is not taxed at source, expenses are claimed under self-employment rules, and you may need to consider payments on account and, in some cases, VAT registration. If you provide locum services through your own limited company, off-payroll working rules may also apply depending on who you engage with. The practical consequences for your record keeping, cash flow and tax returns are significant enough to be worth reviewing properly rather than assuming PAYE is doing the work for you.
There is no universal answer. Incorporation can suit some doctors, particularly where there is substantial private or consultancy income, but it brings additional obligations: statutory accounts, Corporation Tax returns, confirmation statements, payroll and a greater administrative burden. It can also be the wrong choice where off-payroll rules apply to the engagements involved. The decision should follow an analysis of your income mix, how your contracts are structured, your pension arrangements, and how much you value simplicity versus flexibility.
VAT is not automatic, and the position is not the same for every service. Some medical services provided by registered health professionals may fall outside the scope of VAT, while others — including certain non-medical, cosmetic, advisory or product-related activities — may not. Where a private practice supplies mixed services, the VAT treatment can differ across them. We review what you actually provide and advise on registration, record keeping and reporting obligations where they apply.
Yes, and it is often worth looking at the two together. Contributions to the NHS Pension Scheme and to private arrangements can affect the tax relief you receive, and annual allowance questions sometimes arise where earnings are high or have changed significantly during the year. Pension matters also interact with decisions about business structure and how income is drawn. We can review the position with you and flag where specialist pension or financial advice would be sensible.
Switching is usually more straightforward than doctors expect.
