It depends on your circumstances rather than your job title. A salaried doctor whose only income is taxed through PAYE may have relatively straightforward affairs, although a tax return can still be required if you have additional income, benefits or pension complications. Doctors with private practice income, locum earnings, consultancy fees, medicolegal work, property income or partnership profits usually have more to report and more scope for getting the detail wrong. An accountant is most valuable where there are multiple income sources, expenses to substantiate, or a structure decision to make.
Not every doctor does, but many do. You are more likely to need one if you receive untaxed income such as locum fees, private practice or consultancy payments, if you are a partner in a practice, if you have income from savings, dividends or property that is not fully taxed at source, or if HMRC has asked you to file. Employment alone does not automatically remove the requirement. We can review your position and confirm whether a return is needed and what should be included on it.
Locum doctors often have a reasonable set of claims available, but they need to be properly supported. Medical indemnity insurance, professional subscriptions and registration fees, specialist equipment required for your work, and travel between temporary workplaces are examples of costs that may be allowable, provided they meet the rules. Ordinary commuting to a permanent workplace is not. Because the boundaries matter, keeping receipts and mileage records as you go makes a significant difference to how easily a claim can be made.
Yes, and this is one of the most common situations we deal with. NHS income normally reaches you through PAYE, while private and fee income usually does not, so the two need to be brought together to establish your correct tax position. That involves reconciling payslips, pension contributions and private income, tracking expenses against your self-employed or practice work, and making sure any payments on account are planned for rather than absorbed at short notice.
There is no single right answer, and the decision depends on your level of private income, your NHS role, how you are engaged by agencies or hospitals, and how much administration you are willing to take on. A limited company can suit some doctors with substantial private earnings, while sole trader or partnership status is often simpler and perfectly adequate for others. Contractual questions such as IR35 can also arise where locum work is arranged through an agency. We look at your specific circumstances before recommending a structure.
As a minimum, you should retain invoices and fee statements, receipts for expenses you intend to claim, bank and credit card statements, mileage and travel records, pension contribution details and any correspondence with HMRC. If you run a practice, payroll and VAT records will also be needed where they apply. HMRC sets retention periods for business records, and we can confirm what applies to your situation so you are not holding on to paperwork unnecessarily or discarding it too early.
Yes. Much of the accounting work we do for doctors is handled remotely, with documents and records shared securely and meetings held by phone or video at times that fit around clinical commitments. That said, we are happy to discuss how you would prefer to work, since some doctors prefer a mixture of remote communication and in-person meetings, particularly when a structure decision or a complex tax matter is being worked through.
In most cases, yes. Late or missing returns, outstanding liabilities and HMRC enquiries are all situations we can assist with, and the earlier they are addressed the more options tend to be available. We will review what has happened, establish the current position with HMRC, and put a plan in place to bring things up to date. It also helps to speak to us before a deadline passes where you can see one approaching.
Generally easier than people expect. Once you decide to move, we can contact your existing accountant for the relevant records and information, including any professional clearance requirements, and take over the ongoing work. If your previous accounts or returns are already filed, we will review them so we understand your position from the outset. The main thing is timing — it is usually smoothest to make the change outside a busy filing period.
