Not every doctor does. If your only income is a salaried NHS post taxed entirely through PAYE and you have no significant savings income, investments or private work, your tax affairs may be manageable without one. But once you take on locum shifts, private fees, consultancy work, a practice share or a directorship, the picture changes. Self Assessment becomes necessary, expense claims need to be justified, and reporting obligations multiply. An accountant's value is in making sure nothing is missed, nothing is overclaimed, and the tax you pay is the tax actually due.
You generally need to register with HMRC if you have income that is not fully taxed at source. For doctors, that commonly means self-employed locum income, private practice fees, consultancy earnings, untaxed savings or rental income, or high-income child benefit charges. Registration needs to happen by the relevant HMRC deadline for the tax year in question, and the return and any tax due follow on set dates. Because the timing rules matter, it is worth reviewing your position as soon as a new income stream starts rather than waiting until the end of the tax year.
NHS employment income is normally taxed through PAYE, with tax deducted at source and national insurance contributions handled by the employer. Private medical income, locum earnings and consultancy fees are usually self-employed income, which means you report them on a Self Assessment return, pay tax and Class 4 national insurance on the profits, and may need to make payments on account. Because both types can appear in the same year, the return has to bring them together accurately — and the expenses relating to each need to be attributed to the right source.
Common claims include medical indemnity insurance, GMC and specialist registration fees, membership subscriptions to professional bodies, mandatory training and continuing professional development costs, and certain equipment or professional literature. Travel and mileage may be claimable in some circumstances but not others. The rules differ depending on whether an expense is incurred wholly and exclusively for self-employed work, or claimed as an employment expense where your contract requires it. Because the treatment varies, it is worth checking rather than assuming.
There is no single answer. Operating as a sole trader is simpler and involves less administration, but a limited company can offer different tax treatment on profits, more flexibility around income timing, and limited liability. For doctors, the right structure often depends on the level of private income, whether you have NHS employment alongside it, how you want to extract profits, pension arrangements, and your longer-term plans. It is a decision worth reviewing as your private work grows, since what suited the early stages may not suit it later.
Yes. Where doctors work together in a partnership — a GP practice, for example — the partnership itself needs accounts and a tax return, along with a clear allocation of profits between partners. Each partner then reports their share on their own Self Assessment return. Getting the profit-sharing arrangements documented correctly, and keeping the underlying bookkeeping tidy through the year, makes the whole process considerably smoother and reduces the risk of an unexpected tax position for any partner.
VAT is a complex area in healthcare. Many medical services are exempt, but not all activities carried out by a practice or consultancy fall within that exemption, and VAT registration depends on taxable turnover against the prevailing threshold. Where VAT applies, the rules on what can be reclaimed are also specific. Whether registration is required or advisable is a question that needs to be assessed against the actual services you provide, so it is best reviewed directly rather than assumed from a general rule.
Yes — accounting support can be provided remotely, which suits doctors with busy and unpredictable schedules. Documents and records can be shared electronically, and meetings arranged around clinical commitments. If you are moving from another accountant, the process is usually straightforward: we confirm your position, obtain the necessary authorisations with HMRC, and request the relevant records and history from your existing adviser. It is generally best to time a change outside the busiest filing periods, so we can review your affairs properly before the next deadline.
