Not every doctor does. If you are employed on a straightforward salaried contract and all of your tax is collected through PAYE, your affairs may be simple enough to handle yourself. An accountant tends to become valuable when your position is more complex — for example, if you receive private medical fees, locum income or consultancy payments, hold a share in a practice, run a limited company, or need to claim professional expenses such as indemnity cover and subscriptions. In those situations, an accountant can help make sure everything is reported correctly and that legitimate costs are not overlooked.
It depends on your circumstances rather than your job title. Salaried doctors whose tax is fully handled through PAYE may not need to file. However, a return is usually required where you have untaxed income such as private practice fees, locum earnings paid gross, consultancy income, rental income, or significant investment income, and there are also situations where HMRC expects a return even if no additional tax is due. If you are unsure, it is worth reviewing your income sources before assuming you can skip it.
NHS employment income is generally taxed through PAYE, with tax deducted at source. Private medical income is usually different — you may need to report it through Self Assessment and pay tax on it separately, often with payments on account towards the following year. Where you have both, the two are not calculated in isolation; they combine to determine your overall tax position, which can affect the rate applied to your private earnings and any other income you receive.
Allowable expenses are those incurred wholly and exclusively for the purposes of your work. For doctors, common examples include medical indemnity insurance, professional body subscriptions and registration fees, required training and continuing professional development, specialist equipment and reference materials, and business travel or mileage between work locations. What you can claim depends on whether the cost relates to employment, self-employment, or both, so it is worth checking before assuming an expense qualifies.
Locum work is often treated as self-employment, which means you are responsible for reporting your income and paying tax and National Insurance yourself rather than having it deducted through PAYE. That usually involves registering with HMRC, keeping records of earnings, agency statements and expenses, and filing a Self Assessment return. Locum doctors may also need to consider whether their arrangements look like employment in practice, as this can affect how income is treated. If you combine locum shifts with a salaried post, both need to be reflected accurately in your return.
There is no single answer that suits every doctor. Sole trader status is common for locum and small-scale private work and is relatively simple to administer. A partnership is typical where doctors share in a practice's profits. A limited company can suit larger private practices or consultancy income, but it brings additional obligations such as annual accounts, confirmation statements and Corporation Tax. The right choice depends on your income levels, how you want to take money out, your pension position and your future plans, so it is worth reviewing rather than assuming one structure is automatically better.
Doctors frequently work across multiple sites and long hours, so many prefer to handle their accounting remotely. Documents can be shared securely, records kept digitally, and meetings held by phone or video at times that fit around clinical commitments. If you would rather meet in person, that can usually be arranged. Either way, the aim is to make the process convenient rather than adding another appointment to your week.
Changing accountants is usually straightforward. Once you decide to move, we can contact your existing accountant to request the handover of your records, and we will let you know what we need — typically details of your income sources, recent tax returns and accounts, bookkeeping records, and confirmation of any HMRC references. If you are coming up to a filing deadline, telling us early gives more time to review your position properly before anything is submitted.
